Full Time Physical Therapist Assistant
Anticipated Territory: Alameda County: Hayward, Union City, San Leandro*
Under the supervision of a Physical Therapist, you will assist in providing appropriate aspects of quality patient care and address the needs of individuals in their homes, so they can live safely and productively in their home environment.
What you'll do as a 21st Century Home Health Physical Therapist Assistant:
- Examine each individual and develop a plan using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability.
- Work with patients to prevent the loss of mobility before it occurs by developing fitness- and wellness-oriented programs for healthier and more active lifestyles.

To be a success as a 21st Century Home Health Physical Therapist Assistant:
- License PTA in California
- Home Health experience is highly preferred
- Current CPR License
- Current CA Driver's License
- Computer Proficiency
- Strong organization and communication skills
- Work well independently and in a team
- Excel at building rapport with patients and providing the highest quality of care
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired

- Prior to scheduling an in-person interview, all Candidates are required to complete a computer literacy assessment. This is to ensure you are set up for success and can navigate in our environment including electronic charting and use of our web based communication application. The results of the assessment are confidential and will be used as one of multiple data points that at go into the hiring decision process
$85,000 - $160,000 a year
* You will be assigned to a primary location, however you will be expected to be available to cover for vacancies, leave of absences, as well as supporting increased census and client care needs outside of the assigned area. 21st Century Home Health puts forth every effort to accommodate the assigned primary territory, but on occasion, in order to support safe patient care and meet the demands of a growing business, staff may be reassigned temporarily.
