Your Role and Contribution:
- Ensure that our clients receive personalized and effective care
- Conducting comprehensive client assessments and creating initial care plans.
- Regularly auditing and modifying care plans to align with client needs.
- Enhancing client care programs and advocating for clients and their families.
- Maintaining rigorous documentation
- Ensuring caregivers are fully briefed and aligned with care plans.
Our Ideal Candidate:
- Has at least two years of experience in a licensed home care organization.
- Boasts a minimum of two years in a care coordination role within home care or a similar setting.
- A Bachelor’s degree in a related field is preferred but not required.
- Is an excellent communicator, detail-oriented, and empathetic.
$28 - $30 an hour
$28-$30 per hour, based on experience
