Iris Healthcare powered by Aledade, Inc is looking for full time FOCUS Facilitator Supervisor. We are seeking highly self-motivated, outgoing, compassionate, mission-driven professional within our CACP Facilitation team. This role supports the Assistant Director of Clinical Programs in ensuring the organization maintains a high level of quality across the FOCUS Facilitation team.

Job Duties:

    • Supervises FOCUS facilitators as assigned by Director of Clinical Programs
    • Monitors team members’ daily activities and responds to questions in a timely manner.
    • Monitors team members’ schedules and attendance to ensure calendar coverage for patients.
    • Monitors & supports team's data integrity.
    • Clinical Coaching, Supervision, communication.
    • Reviewing/approving patient documents in a timely manner (24-48 hours)
    • Evaluates team members’ performance (both clinical and administrative) on an ongoing basis to ensure they meet company goals and expectations.
    • Conducting 1:1, dyadic and group supervision and coaching sessions as appropriate to support staff with performance goals (both clinical and administrative).
    • Ensure all team members maintain and conform to both Aledade and Iris CACP standards, policies, and HIPAA.
    • Actively support FOCUS Team as a whole through glip/email
    • Maintain active practice as an Iris Facilitator as able given size of team through role play, supporting SOS/Same Day Facilitations.
    • Leads by example (communication, ethics and clinical quality)
    • Collaborates with Facilitation Supervisory Team, Assistant Director of Clinical Programs, and Director of Clinical Programs to ensure facilitation quality and consistency of guidance and messaging.
    • Identifies training needs and coordinates training resources as needed, including offering trainings as appropriate


    • License as a Nurse, Social Worker or Counselor.
    • Demonstration of proficiency in the Iris Facilitator role (clinical quality and documentation)
    • Supervisory experience a plus, but not required
    • Must be in current role for 6 months or more.
