Aledade Care Solutions of North Carolina is looking for a full-time Pharmacist to join our clinical care team to support Aledade Care Solutions, Inc. Aledade Care Solutions of North Carolina delivers new clinical service lines and business offerings that will augment and extend Aledade’s primary care practices’ clinical capacity, offering selected patients a portfolio of virtual and (eventually) home-based services to improve health at lower costs.

Our market-based interdisciplinary teams manage wrap-around care for patient panels who have been identified for comprehensive care management in collaboration with patients’ PCPs.  These teams are led by Physicians and Nurse Practitioners with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, and other employed team members.

Position Responsibilities

    • Review, reconcile and resolve medication-related problems and make appropriate evidence-based, patient-centered medication and therapy recommendations as part of the interdisciplinary team.
    • Conduct thorough medication reconciliations for patients post-hospital discharge
    • Conduct comprehensive medication reviews (CMRs) for qualified patients
    • Identify and address patient-specific barriers to medication adherence and chronic condition management
    • Provide clinical decision support, education and consults to ACS providers and practice PCPs when needed. 
    • Provide direct-to-patient education and self-management for medication efficacy
    • Enhance coordination of care between care managers, providers and pharmacies to ensure appropriate medication management for patients. 
    • Collaborate with providers, clinical and administrative staff to optimize the Medication Management workflow and patient experience.
    • Adjust and recommend alternative pharmacotherapy / therapeutic substitutions based on patient formulary and prescription cost
    • Monitor aggregated data sources to determine appropriate patients for medication management intervention in addition to provider referrals
    • Approximately 25% local travel may be required for practice engagement.

Required Licensure and Experience:

    • Graduate of an accredited School of PharmacyDoctor of Pharmacy (PharmD) degree [preferred] or Bachelor’s degree in Pharmacy
    • Current, unrestricted pharmacist licensure in the state of North Carolina
    • Strong communication skills with patients, providers and other healthcare professionals
    • 3+ years of experience in a clinical pharmacy setting
    • In-depth knowledge of pharmacotherapy and pharmacy practice
    • Ability to work with multiple software applications


    • PGY-1 or PGY-2 ambulatory pharmacy residency preferred
    • Experience with Medication Therapy Management
    • Basic understanding of Medicare & Medicare Advantage quality measures, with Commercial quality measures being an additional plus. 
    • Familiarity with virtual care delivery and wrap-around care management services within managed care and/or value-based practice 
    • Familiarity with a metrics-driven approach to analyze cost, quality and satisfaction data
