Aledade is seeking a Director of Community Health Center Performance. The Director will be responsible for developing and maintaining data systems to track contract performance for our CHC partners to support high quality and equitable care for the patient populations seen at CHCs. Using this data system, the Director will support performance management of CHC partners in Aledade ACO networks by surfacing opportunities for improvement based on data. 

This role requires experience working with Federally Qualified Health Centers or prior experience holding a leadership role at an FQHC, as well as understanding population health analytics and value-based care principles. The Director of CHC Performance will understand the environment and complexities under which CHCs provide value-based care (stakeholders, policies), and will be able to lead the development of new programs and resources for CHC performance improvement based on data analytics and his/her comprehensive understanding of CHCs. 

The Director of CHC Performance will be a key leader for the Aledade CHC Center of Excellence, designed to enhance Aleadade’s value-based care support to CHCs by developing and managing programs that  training, educate and facilitate VBC success for CHCs (affordability, risk, quality metrics), as well as adapting Aledade’s core service offerings for CHCs, all through a community-centric approach. 

The Director is expected to work closely with leaders from other teams at Aledade to develop strategies and solutions that support organizational key result (KR) performance outcomes, and s/he must also be comfortable speaking about CHC performance in settings with CHC leaders. 

This role works with both internal and external partners, and requires strong organizational, communication, and analytical skills. The Director will need to synthesize feedback and comments and translate the information into actionable next steps to ensure the successful execution and achievement of program goals. 

We are flexible with respect to geographic location, and the ideal candidate will be comfortable working remotely within the US, with periodic travel required for internal planning meetings and in-person CHC events.

Primary Duties:

    • Collaborate with stakeholders to track performance (affordability, risk, quality, pop health impact) for CHCs on ACO performance, including presenting information at internal Steering Committees. 
    • Develop and manage CHC data dashboards with support from BI, and create visibility on CHC metrics
    • Leverage data insights to develop and execute on strategies to ensure VBC success via the CHC Center of Excellence 
    • Support payer strategies designed for CHCs based on data
    • Participate in Collaborative Learning Networks, including the development of standard curriculums
    • Represent Aledade to CHCs during CHC summits and events 
    • Other duties as assigned to ensure maximal coordination across Aledade teams.

Minimum Qualifications

    • Bachelor’s degree and/or 10+ years of healthcare work experience
    • 2+ years experience working with Federally Qualified Health Centers or in a Federally Qualified Health Center
    • 5+ years of progressive value-based care experience
    • Demonstrated ability with population health analytics, including data systems and data visualization 
    • Able to thrive in a fast-paced, startup environment
    • Demonstrated experience with taking an idea from concept to execution
    • Proven ability to successfully manage multiple programs, coordinating and collaborating across projects and teams.
    • Strong and clear thinker/writer. 

Preferred Qualifications:

    • Expertise in FQHC business models, such as billing/coding strategies and FQHC operations 
    • Leadership level experience in FQHC strategy and population health initiatives.
    • Knowledge/background participating or leading IHI-style collaboratives
    • Skilled with Tableau
    • Master’s degree or other advanced degrees in business, healthcare administration, or public policy.

Physical Requirements:

    • Sitting for prolonged periods of time. Extensive use of computers and keyboard. Occasional walking and lifting may be required.
    • Willingness to travel 10-15% of the time, as needed, to support CHC events.
