As a CHC Performance Management Specialist at Aledade, you'll be part of our CHC Center of Excellence Team, directly supporting Community Health Centers as they navigate complex business challenges and optimize operations for value-based care success.
Your FQHC experience will be essential as you analyze performance data, conduct workflow assessments, and identify improvement opportunities for our CHC partners. You'll collaborate with the COE team to develop targeted resources and processes that boost performance on critical metrics like annual wellness visits, patient attribution, risk adjustment, and Stars ratings.
This role combines analytical expertise with program management - you'll facilitate COE meetings and events, coordinate cross-functional projects, and translate insights into actionable recommendations that help safety-net providers thrive while serving underserved communities.
You'll see direct impact as your work helps Community Health Centers strengthen their operations, improve patient outcomes, and succeed in value-based care arrangements that benefit both providers and the communities they serve.
We are flexible with respect to geographic location, and the ideal candidate will be comfortable working remotely within the US, with periodic travel required for in-person CHC consulting, CHC events, and internal planning meetings.
Primary Duties:
- Analyze data, identify opportunities for improvement, and follow up on building a consulting plan for identified CHCs, in partnership with COE team members Manage the pipeline of CHCs in different consulting phases (initiation, adoption, execution, close-out)
- Manage multiple COE events, including consulting efforts, planning for Collaborative Learning Events, and developing solutions (e.g., process maps, training materials, etc), create resources, and present information in either oral or written format).
- Coordinate/collaborate with CHC Center of Excellence Staff on building program, services, and resources for CHCs and marketing that information to both internal and external stakeholders.
- Coordinate consulting services with COE team members and other Aledade Field Staff on meeting logistics, and follow ups
- Represent Aledade to CHCs during CHC summits and events
- Other duties as assigned to ensure maximal coordination across Aledade teams.
Minimum Qualifications:
- Bachelor’s degree in a related field with 3+ years of professional experience.
- 2+ years experience working with or in an FQHC
- 2+ years of progressive value-based care experience
- Able to thrive in a fast-paced, startup environment,
- Demonstrated experience with taking an idea from concept to execution
- Proven ability to successfully manage multiple programs, coordinating and collaborating across projects and teams.
- Strong and clear thinker/writer.
Preferred KSA's:
- Demonstrated experience using Tableau or other analytics tools
- Change Management experience (e.g., Lean Six Sigma)
- Master’s degree or other advanced degrees in business, healthcare administration, or public policy.
Physical Requirements:
- Sitting for prolonged periods of time. Extensive use of computers and keyboard. Occasional walking and lifting may be required.
- Willingness to travel up to 50% of the time across US markets
$75,000 - $90,000 a year
Salary Range: $75,000 to $90,000 base + bonus + equity
Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience.
