About the Position:
The Youth Employment Initiative Career Coach/Case Manager provides direct case management and career development services to justice-involved youth with disabilities and will be embedded within the County of Los Angeles Probation Department Secure Youth Treatment Facility (SYTF). Responsibilities include conducting intake assessments, developing individualized case plans, monitoring participant progress, coordinating closely with community-based organizations, mentors, and partner agencies, and ensuring successful transition into employment or further education. The role requires frequent data entry and careful tracking of participant outcomes through the Apricot data system. This position emphasizes cross-sector collaboration, proactive family and community engagement, and ongoing adjustments to programming based on outcome data. 

What You Will Do:

    • Essential Duties:
    • Conduct comprehensive intake assessments for assigned students, identifying strengths, needs, and barriers.
    • Develop, review, and regularly update Individual Development Plans (IDPs) tailored to each student’s goals, emphasizing employment, education, and supportive services.
    • Provide personalized mentoring, including weekly one-on-one and group sessions focused on soft skills, motivational interviewing, problem-solving, crisis management, and trauma-informed care.
    • Coordinate closely with Workforce Development Navigators and other case management personnel stationed at Secure Youth Treatment Facilities (SYTFs), including Barry Nidorf Juvenile Hall (Sylmar), Campus Kilpatrick (Malibu), and Dorothy Kirby Center (Commerce).
    • Support students in acquiring employment skills, preparing resumes, practicing interview techniques, and securing job placements.
    • Facilitate student’s enrollment in educational programs and assist with financial aid applications when applicable.
    • Document all progress, services provided, and employment outcomes thoroughly in the student’s files and the Apricot data management system.
    • Regularly meet with mentors to align mentoring activities with student IDPs.
    • Monitor student progress and regularly coordinate with Community-Based Organizations (CBOs), Probation, Department of Rehabilitation, local workforce development boards, and the County Office of Education regarding case planning and transitions.
    • Develop discharge and post-program follow-up plans, ensuring continuous support for sustained success.
    • Make appropriate referrals to external agencies for additional support services as necessary.
    • Maintain comprehensive case records and accurate statistical data for students ensuring documentation meets all compliance requirements.
    • Regularly review student’s outcome data with the YEI Project Coordinator to make timely adjustments that enhance program effectiveness.
    • Actively engage and communicate with students' families or caregivers to support a holistic approach to participant development and employment sustainability.
    • Participate actively in community-building events, collaborative stakeholder meetings, workshops, and relevant training.
    • Maintain effective communication and collaborative relationships with the YEI Project Coordinator, stakeholders, and community partners.
    • Serve as a community advocate as requested by program leadership.
    • Perform other related duties as assigned.

    • Recognition of the importance of individual differences created by personalities, cultures, lifestyles, social networks, and other factors that influence behavior.
    • Operational knowledge of MS Word, Excel, and Access
    • Ability to prioritize competing tasks with multiple deadlines.
    • Excellent interpersonal and communications skills and the ability to work effectively with a wide range of constituencies in the community and within the organization.
    • Demonstrate ethical behavior to maintain professional standards and safeguard the participant.
    • Participate in community-building activities as appropriate to the position.
    • Protect program participants' health, safety, welfare, and confidentiality of records and participant information.
    • As a condition of employment with Amity, a job offer will not be binding upon Amity until and unless Amity and the LA County Probation Department  obtain satisfactory results from its investigation into your background

    • Education and Experience:
    • Required:
    • Bachelor’s degree in social sciences, youth development, rehabilitation counseling, workforce development, or a related field; OR
    • High school diploma or equivalent with a minimum of two years’ experience working with justice-involved youth, individuals with disabilities, or related populations.
    • Preferred:
    • Experience providing case management or career coaching services within juvenile justice or rehabilitation settings.
    • Certifications or Licenses:
    • Required:
    • Valid California driver's license and eligibility for Amity vehicle insurance.
    • Preferred:
    • Certification in motivational interviewing or trauma-informed care practices.
