Key Responsibilities:
- Schedule and manage all laboratory tests.
- Manage and monitor toxicology reports, including both random and planned assessments.
- Document and monitor vital signs accurately.
- Assist prescribers and RNs in obtaining pre-authorizations for medication refills.
- Advocate for the care of individuals served, addressing psychiatric and somatic needs as required.
- Collaborate with pharmacies to secure necessary medications, supplies, and durable medical equipment.
- Coordinate care with physicians, hospitals, pharmacies, and nursing facilities, focusing on psychiatric, somatic, and medicinal needs.
- Support Senior Program with various assigned tasks.
- Manage the AACHC schedule and communicate with AACHC leads and provider designees for REMS and medication-related matters.
- Conduct audits of medication boxes and medication management records (MMR).
- Perform medication reconciliation and complete Health Home notes as applicable.
- Assist with agency-wide admissions and discharges for residential and hospital services, both psychiatric and somatic, and provide individual consultation as needed or requested for Senior Program and community clients.
- Oversee office management tasks, including supplies and equipment.
- Manage care alert notifications.
$35 - $37 an hour
Position Overview:
The RN Senior Program Nurse will liaise with clients, family members, healthcare providers, and insurance companies. This role is essential in coordinating comprehensive care and ensuring effective communication across all parties.
RN License Required!
Part Time: 20 hours, 3 days per week, flexible Mon-Thursday (choose one)
Hours: M-TH pick 1/ 1:30pm-6:30pm, (Mandatory Friday (9am-5pm) & Saturday (9am-4pm)
Qualifications:
Education: Bachelor of Science in Nursing (BSN), Associate's degree in Human Services, or equivalent Registered Nurse credential. Experience: At least one year of experience in Human Services is preferred, particularly as a Registered Nurse.
