Bodyport is a virtual cardiac clinic aimed at reducing the burden of heart disease through personalized, evidence-based care. In just one daily step, our novel sensors and algorithms predict changes in health status, enabling early, optimized interventions that significantly improve outcomes. Bodyport is initially targeting heart failure, a chronic condition affecting over 6 million Americans and a leading cause of hospitalizations (1M/year) and healthcare spending ($35B/year). As a Bodyport Care Coach you will be our patient’s initial point of contact in caring for their heart health. Your responsibilities will include interacting with patients to obtain routine clinical information, providing support with scale use, and reviewing heart failure educational materials. You will also be responsible for appropriately triaging questions or concerns to medical providers when necessary. We are looking for highly motivated, enthusiastic, and patient-focused individuals to join our mission-driven team.

Your role at Bodyport:

    • Serve patients flexibly, efficiently, and effectively to drive the best outcomes for them
    • Onboard new patients to the Bodyport Cardiac Platform.
    • Develop and grow a relationship with patients, covering topics including education, Q&A, and leveraging motivational techniques to encourage optimal health habits
    • Daily triaging of patient-activated alerts, including outreach to patients for additional information about their status. 
    • Escalation of patient needs to health care providers when patients are expressing concerning symptoms.
    • Be familiar with the care protocols and the Bodyport Cardiac Platform to address patients questions as they arise. 

You'd be a good fit if you have:

    • A 4-year college degree is required in one of the following areas: Nursing, Social Work, Psychology, Nutrition, Dietetics, Kinesiology, Exercise Science, or other health-sciences related field. 
    • Prior patient experience in one of the following areas: health coaching, health counseling, mental health care, motivational interviewing, patient interviewing using behavior changing techniques, etc. 
    • Outstanding verbal and communication skills. 
    • Access to a solid internet connection and technology literacy with online communication platforms (i.e., text, email). 
    • A growth mindset, collaborative personality, and unquestionable integrity. 
    • Strong desire to work directly with patients and provide top level care. 
    • Willingness to reach out to teammates if any questions arise, fostering a safe working environment that promotes support when needed.
