The Opportunity

Position Summary:
Responsible for assisting clients and families with social and financial issues related to continuing care admissions or return to the community. Collaborating with the Community Care Coordinator, Transition Services to assist with the development of the discharge plan. Communicating with the client and family about progress toward the appropriate discharge location.

Program Summary:
Rehabilitation and Community Transition Program (RCTP) – a short-stay restorative care program that supports medically stable
adults who do not need an acute-care level of service, but may require additional assessment, recuperation, and therapy before
returning to the community.

Qualifications & Experience

    • Graduation from a recognized degree program in Social Work or equivalent program
    • Registered with Alberta Association of Social Workers;
    • Valid Driver’s License
    • Minimum of 2 years experience in a combination of clinical and continuing care roles; Gerontology experience is preferred

    • Rate of Pay: $ 38.56 - $ 51.27
