Wage: 25.13 / hr + DOE + DOQ
Shift: Monday - Friday

Qualifications:

    • Associate degree in human services, social work, sociology preferred. Can substitute years of experience for degree requirements.  
    • Two years of case management or social services experience; and/or equivalent combination of education and experience preferred. 
    • Knowledge of and experience in using trauma-informed care principles required.  
    • Demonstrated experience in mediation between personsexhibiting escalated behavior required. 
    • Must have valid Oregon Driver's license; must have been a licensed driver for at least 2 years; must have a driving record that permits coverage under C@P’s company insurance (required). 

Essential Job Duties Include (But Not Limited To):

    • This is a mobile case management position that requires working across multiple locations in and around Salem, including shelters, encampments, public spaces, partner agencies, and community-based settings. Flexibility, adaptability, and comfort working in dynamic indoors and outdoors environments are essential. 

    • Case planning and follow through: 
    • Make appropriate referrals for drug and alcohol treatment, domestic violence services, and health services. 
    • Assist participants in obtaining vital documents such as birth certificates, identification, Oregon Health Plan enrollment, free phones, and transportation vouchers. 
    • Support management in developing action plans to address challenging behaviors and participant disengagement. 
    • Connect participants to workforce development workshops and related resources. 
    • Conduct intake assessments with new Safe Parking participants, gathering essential information to support service planning, ensure eligibility, and connect individuals with appropriate resources. Ensure that intakes are completed in a respectful, trauma-informed, and participant-centered manner. 
    • Track services needed by each participant and assist in developing goals and action steps. 

    • Identify and Breakdown barriers to obtaining Medical Insurance/Services: 
    • Develop a system to ensure OHP enrollment and renewal of guests across C@P operated sites and programs.   
    • Help secure transportation to medical appointments (transport, bus, medical transport van) as needed. 
    • Assist guests in connecting with an OHA assistor to obtain medical insurance as needed. Have a more in-depth knowledge of the OHA medical system.  
    • Maintain all PHI so HIPAA-compliant standards are met. 

    • Collaboration and Communication.
    • Maintain regular communication with C@P staff to ensure coordinated care. 
    • Uphold policies and procedures while maintaining healthy boundaries and respectful relationships with guests and colleagues. 
    • Attend required training and team meetings and stay informed of C@P operations policies and procedures. 
    • Work in partnership with C@P Health Systems Navigators to facilitate the navigation of medical services. 
    • Collaborate and communicate regularly with church partners who host Safe Parking participants to support positive relationships, ensure program expectations are upheld, and address concerns as they arise. Service as a respectful liaison between participants and church hosts, promoting mutual understanding, conflict resolution and a sense of shared mission. 
    • Facilitate connections to resources to assist residents in exiting to permanent or other positive destinations. 
    • Collaborate with Salem Housing Authority, ARCHES, and other service providers to support participants in receiving housing assessments and updating Coordinated Entry information for emergency or permanent housing placement. 
    • Communicate and work closely with internal and external partners to ensure participants have a continuity of care. 

    • Data Entry and Documentation: 
    • Accurately maintain participant files and service data in HMIS, ActivateCare, and other relevant systems. 
    • Ensure data quality and timely entry to support effective case tracking and reporting. 
$25.13 - $25.13 an hour
Work environment: 
Close quarters with vulnerable people, including individuals without stable living situations who may be struggling with mental health and substance use concerns.  
Indoor/Outdoor work environment with frequent interruptions and demands, and occasional unpredictable behavior.  
Frequent walking through camps, which may have unstable surfaces (mud, roots and rocks on ground). This position requires walking long distances outdoors in adverse weather conditions. 
Must be able to push, pull, and/or lift 50 pounds or less.   
Must be able to go up and down stairs. 


Transportation Policy:

To receive and maintain authorization to drive a Non Profit vehicle, a driver must:  

Drivers must have a valid driver’s license of the state they reside in. Employees who are hired with an out of state license will be given 30 days (subject to DMV availability) to acquire a license for the state in which they reside.
Drivers should have at least (2) years driving experience and be at least 18 years old.  

In the past three by (3) years, drivers should have no more than: 

Two (2) At-Fault Accidents, (proof of not-at-fault status for accidents must be received to rescind this. Your personal auto insurer or DMV can assist with this information.
Three (3) Minor Moving Violations, (personal and/or business) 
Four (4) Non-Moving Violations, such as: Failure to Appear (FTA), License not in possession, or No Proof of Insurance/Registration. 
Four (4) or more in combination of the above a, b, and c.  

In the past four (4) years, drivers should have zero (0) Major Moving Violations such as: 

Driving while Suspended/Revoked and/or Invalid License Exhibition of Speed - Speed Contest,
Reckless Driving,
Driving Under the Influence (DUI) 
Vehicular Manslaughter,
Leaving the Scene of an Accident (Hit and Run) 
Speeding in excess of 100 mph, etc.
