As a Nurse Practitioner (NP) in our Remote Patient Monitoring team at Circadia Health, you will assume a pivotal role in remotely overseeing and evaluating patient vital signs and Electronic Health Records (EHR) data for individuals residing in Skilled Nursing Facilities, utilizing our advanced remote monitoring system. Operating under the supervision and guidance of a Registered Nurse (RN) who leads the team, your expertise and meticulous attention to detail will be instrumental in the early detection of changes in patient conditions, thereby facilitating timely interventions and the enhancement of patient care.

Key Responsibilities:

    • Clinical Decision Support: Utilize clinical decision support tools and algorithms to assist in identifying patients at higher risk and recommend appropriate interventions.
    • Patient Advocacy: Advocate for patients' needs within the healthcare system, ensuring they receive the appropriate level of care and services.
    • Quality Assurance: Participate in ongoing quality assurance and improvement initiatives to enhance the effectiveness of remote patient monitoring and care delivery.
    • Data Analysis: Analyze trends and patterns in patient data to identify opportunities for improving monitoring protocols and patient care.
    • Documentation and Reporting: Maintain accurate and thorough patient records, documenting all assessments, interventions, and communication with patients and healthcare team members.
    • Training and Development: Stay updated on the latest advancements in remote patient monitoring technology, healthcare guidelines, and best practices through ongoing training and professional development.
    • Interdisciplinary Collaboration: Collaborate with interdisciplinary healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure seamless patient care coordination.
    • Remote Patient Monitoring: Thoroughly review vital signs and EHR data of patients in Skilled Nursing Facilities through our FDA-Cleared contactless patient remote monitoring system.
    • Patient Condition Documentation: Document changes in patient condition, with oversight from an RN, based on observed trends in vital signs data.
    • Risk Assessment Collaboration: Collaborate under RN supervision to assist in defining patient risk levels based on remote monitoring data and individual patient parameters outlined by the RN.
    • Digital Reporting: Generate digital reports for clinicians at Skilled Nursing Facilities, providing valuable insights and data for informed decision-making.
    • Remote Collaboration: Work remotely in tandem with our healthcare team, including the RN, product managers, and clinical implementation specialists, to ensure optimal patient care and monitoring.
    • Adherence to Regulations: Ensure strict compliance with all relevant healthcare regulations, ethical guidelines, and company policies when handling patient data.

Qualifications:

    • Ability to thrive in a fast-paced environment with a startup mentality.
    • Ability to travel within the assigned territory.
    • Previous experience working as a Nurse Practitioner (NP) with licensure.
    • Preferred experience with Electronic Health Records (EHR), e.g., PointClickCare.
    • Exceptional critical and analytical thinking skills.
    • Strong verbal and written communication skills.
    • Familiarity with medical terminology and clinical decision support.
    • Preferred experience using GSuite: Gmail, Docs, Sheets, Slides.
    • Team player with a positive attitude.
    • Proficiency in listening, comprehending, and processing detailed information.
    • Commitment to continuous learning and clinical improvement.
