Position Overview:
We’re looking for a highly organized and empathetic Home Health Patient Coordinator to join our team and serve as a crucial link between our technology and the patients who need it most. In this role, you’ll be the primary point of contact for patients beginning their journey with Circadia’s in-home monitoring technology. You will be responsible for patient outreach and education, answering questions, and scheduling the installation of our monitoring devices with our field operations team. The ideal candidate has exceptional communication skills, a passion for helping others, and the ability to manage a high volume of outreach with professionalism and care. You should be confident working with digital tools, explaining new technology in a simple, supportive way, and ensuring every patient feels comfortable and informed. Key Responsibilities
- Conduct outreach calls to newly referred patients or their caregivers to introduce the remote monitoring program.
- Educate patients and their representatives on the Circadia program, explaining the benefits and purpose of the monitoring service in an easy-to-understand manner.
- Ensure patients understand how their health data will be monitored remotely by our clinical team.
- Answer basic patient or caregiver questions and triage complex inquiries to appropriate team members.
- Collect and document key demographic and scheduling information in our CRM system.
- Coordinate appointment scheduling with the field operations team for in-home device installations.
- Maintain timely and accurate records of all patient outreach, interactions, and scheduling status.
- Support continuous improvement efforts by identifying trends or barriers in the outreach process.
Qualifications
- Previous experience in a customer-facing, coordination, or healthcare-related role is a plus, but not required for candidates with the right transferable skills.
- Excellent verbal communication skills, with the ability to explain new concepts clearly and build trust over the phone.
- Strong organizational skills and comfort using digital tools (CRM, scheduling platforms, etc.).
- Personable and patient-focused, with a dedication to providing a high-quality, supportive experience.
- Ability to think on your feet and problem-solve independently.
- A strong desire to learn and adapt in a fast-paced, evolving field.
