Position Description:
Circadia Health is revolutionizing remote patient monitoring for post-acute care and skilled nursing facilities. We are seeking a dedicated and experienced Nurse Practitioner to join our dynamic team. This role will be pivotal in assessing patients health status and providing care plans while utilizing cutting-edge technology to monitor and improve the health outcomes of our long-term care patients.
As a Nurse Practitioner with Circadia Health, you will play a crucial role in our mission to enhance patient care through innovative solutions. You will perform in‑person assessments and create care plans for long‑term‑care patients while leveraging Circadia’s AI insights. The role is field‑based: you’ll travel to skilled‑nursing facilities across New York (all travel reimbursed) and collaborate with on‑site teams and Circadia clinicians.
This is a per-diem position with flexible hours, with both weekday and weekend availability.
Key Responsibilities
- In-Person Comprehensive Assessments: Conduct detailed patient evaluations face-to-face for current remotely monitored patients. Gather medical histories and assess current health conditions using both direct interactions and AI-powered insights to spot trends and predict issues.
- Facility Visits: Travel to designated local skilled nursing facilities to support patient care and the facility's care team.
- Personalized Care Planning: Develop tailored care plans. Define specific health monitoring parameters, set realistic health goals, and use predictive analytics to refine your clinical decisions.
- Accurate Documentation & Compliance: Keep up-to-date, precise records of patient interactions, assessments, and care plans in the EHR system. Ensure all documentation meets regulatory standards and policies.
- Interdisciplinary Collaboration & Education: Coordinate with facility clinical teams to ensure seamless hand-offs and follow-through on escalations and interventions. Provide just-in-time education to SNF staff on Circadia’s platform and AI insights
KPIs
- Complete a minimum of one visit per hour worked to ensure compliance with regulatory standards.
- Maintain accurate and up-to-date documentation for each patient visit.
Qualifications
- Certified and licensed as a Nurse Practitioner in NY.
- Medicare participation and ability to have company bill for services on your behalf
- Certification from ANCC (or equivalent) as an Adult, Family, Geriatric and/or Acute Nurse Practitioner
- 2+ years of Nurse Practitioner Experience working with complex adult populations
- Ability to practice independently with little or no supervision
- Current NY Driver’s License
- Comfort using technology such as Google Suite, multiple EMRs, Slack, AI transcription tools
- Comfortable in a dynamic and always evolving start up environment
- English required, Spanish preferred
Compensation
This is a pay-per-visit role. Compensation will be based on each completed and signed clinical note in our system. Most nurses are able to complete multiple visits per hour, allowing for competitive earnings based on efficiency and availability.
Rate: $26/completed visit
Onboarding/Meetings: Time spent on onboarding or attending meetings that are pre-approved by your manager will be compensated at a fixed rate of $62.50/hr.
