Position Description:

Circadia Health is revolutionizing remote patient monitoring for post-acute care and skilled nursing facilities. We are seeking a dedicated and experienced Nurse Practitioner to join our dynamic team. This role will be pivotal in assessing patients health status and providing care plans while utilizing cutting-edge technology to monitor and improve the health outcomes of our long-term care patients.

As a Nurse Practitioner with Circadia Health, you will play a crucial role in our mission to enhance patient care through innovative solutions. You will be responsible for performing in-person patient assessments, developing individualized care plans and collaborating with our interdisciplinary team to ensure the highest standards of care. You'll have the opportunity to travel to designated skilled nursing facilities within your region, fostering direct engagement with patient care and on-site teams.

Key Responsibilities

    • In-Person Comprehensive Assessments: Conduct detailed, one-time patient evaluations for current remotely monitored patient within skilled nursing facilities. Gather relevant medical history, perform assessments, and document thoroughly, with support from AI-enabled summaries.
    • High-Volume Facility-Based Rounding: Perform efficient, high-quality visits at designated skilled nursing facilities, targeting 25+ visits/day where appropriate. Travel within a 30–40 mile radius; mileage reimbursement and hotel coverage provided as needed. Focus on new admissions, facility-flagged patients, and Circadia-flagged alerts.
    • Personalized Care Planning: Create and document tailored care plans that reflect the patient’s condition, facility inputs, and Circadia predictive analytics. Educate care teams when needed.
    • Facility Clinical & Circadia Education: Support the re-education of SNF staff about Circadia’s system. Explain the predictive trend-based model and correct misconceptions. Walk staff through patient vitals and trends in the CIP app. Help staff interpret alerts and use SBAR workflows correctly.
    • Documentation & Compliance: Ensure proper and timely documentation including patient consent, complete HPI/PE, medication/allergy reconciliation, and clinical reasoning in the A/P. Collaborate with chart auditors and comply with organizational SOPs.
    • Clinical Product Feedback & Quality Insights: Provide feedback from the field to the product and engineering teams. Participate in root cause analyses on false negatives/positives and missed care opportunities. Contribute to improving app-based workflows, visit summaries, and flagging logic.
    • Peer Support & Onboarding Participation: Mentor new NP hires and support peer development by sharing workflows, best practices, and patient education strategies. Lead by example with performance and collaboration.
    • Team Collaboration & Strategy: Participate in virtual and in-person team meetings, offsite, and strategy sessions to shape the future of proactive senior care. Engage with clinical operations and product teams to share feedback and ideas.
    • Regional Flexibility & Expansion Support: Willingness to temporarily expand regional coverage to support new market launches and help address visit backlog. Prioritize team-based goals and flexibility.

KPIs

    • Minimum of 25 patient visits per day or 500 visits per month.
    • Complete all visit documentation using AI - powered tools within 24 hours of each encounter.

Qualifications

    • Certified and licensed as a Nurse Practitioner in CT
    • Medicare participation and ability to have company bill for services on your behalf
    • Certification from ANCC (or equivalent) as an Adult, Family, Geriatric and/or Acute Nurse Practitioner
    • 2+ years of Nurse Practitioner Experience working with complex adult populations
    • Ability to practice independently with little or no supervision 
    • Current CT Driver’s License  
    • Comfort using technology such as Google Suite, multiple EMRs, Slack, AI transcription tools 
    • Comfortable in a dynamic and always evolving start up environment
    • English required, Spanish preferred
