As a Field Nurse - Home Health your responsibilities will include conducting patient assessments, offering home health education, and actively participating in remote patient monitoring of vitals for individuals within their homes or community environments. The ideal candidate possesses excellent communication skills, adaptability, and a strong sense of empathy. You will collaborate with interdisciplinary teams to ensure comprehensive and coordinated care, while also maintaining accurate and thorough patient records.




Key Responsibilities:

    • Care Coordination and/or Chart Review: Implement effective care coordination strategies and engage in thorough chart reviews to ensure comprehensive and well-coordinated healthcare delivery.
    • Remote Monitoring Implementation: Utilize cutting-edge technology and AI to implement the Circadia Health remote monitoring system, focusing on early detection of medical events such as Congestive Heart Failure, COPD Exacerbations, Pneumonia, Sepsis, UTIs, and Falls.
    • Assessment and Collaboration: Work collaboratively with a multidisciplinary team to assess patients' health conditions, develop comprehensive care plans, and implement strategies to enhance their overall well-being.
    • Clinical Excellence: Demonstrate strong clinical skills to address patient needs effectively, including the ability to interpret and respond to remote monitoring data, ensuring timely interventions when necessary.
    • Communication and Education: Communicate effectively with patients, families, and healthcare providers, providing education on the benefits of remote monitoring and promoting patient engagement in their care.
    • Technology Integration: Embrace the use of technology to streamline healthcare processes, improve patient outcomes, and contribute to the advancement of healthcare practices.
    • Team Collaboration: Collaborate with colleagues, including healthcare providers, support staff, and technology specialists, to deliver seamless and integrated patient care experiences.

Qualifications:

    • Ability to thrive in a fast-paced environment with a startup mentality.
    • Willing to travel within the assigned territory.
    • Experience with EPIC Electronic Health Records (EHR).
    • Exceptional critical and analytical thinking skills.
    • Strong verbal and written communication skills.
    • Familiarity with medical terminology and clinical decision support.
    • Preferred experience using GSuite: Gmail, Docs, Sheets, Slides.
    • Team player with a positive attitude.
    • Proficiency in listening, comprehending, and processing detailed information.
    • Commitment to continuous learning and clinical improvement.
    • Licensed as a California Registered Nurse or Licensed Vocational Nurse.
