The Registered Nurse plays a critical role in delivering high-quality, compassionate healthcare to homeless and at-risk individuals. This position requires clinical expertise within an integrated, trauma-informed care model, focusing on the complex health needs of a diverse patient population. The RN is essential in promoting patient-centered care and improving overall well-being through adaptability, resilience, and a proactive approach to overcoming healthcare barriers.

This health outreach role focuses on health outcomes within an integrated outreach team. As part of the Micro Community grant, this role focuses on improving access to people experiencing homelessness who are now residing in one of the established micro communities. The RN is an integral part of Community Health services. The nurse is responsible for creating and maintaining an active care management process for patients living in the non-congregant shelter they are assigned. The nurse works collaboratively with an interdisciplinary team to support individuals in their transition to a stable housing environment. The nurse provides services in the community within the framework of professional nursing ethics and state and city regulatory guidelines. This is a field-based position and requires at least 80% of services to be provided to consumers in their transitional shelter, temporary or permanent clinic space and other community locations. The nurse provides direct nursing services to consumers of Community Health.

Hours are generally 7:00 am – 5:00 pm M-F, totaling 40 hours. Shifts can be four 10’s or 5 8’s.

Essential Job Functions 

    • Provide skilled nursing care according to community standards and CCH protocols.
    • Deliver comprehensive patient-centered care, addressing physical, emotional, and social needs.
    • Conduct rapid and accurate triage assessments, providing appropriate care recommendations.
    • Educate patients on managing chronic and acute health conditions.
    • Administer medications safely and effectively as per protocols.
    • Facilitate access to internal programs such as the Stout Street Health Center, the Stout Street Recuperative Care Center and housing support services.
    • Oversee care for patients with transitional, chronic, and complex health conditions.
    • Mentor new nurses and nursing students, enhancing their clinical skills and confidence.
    • Ability to perform quick and accurate triage assessments with appropriate recommendations of care as outlined in SSHC nursing protocols via telephone or in-person
    • Collaborates with other members of the multi-disciplinary health care team and coordinates care with internal programs.
    • Provide nursing services as needed to all CCH programs and facilities.
    • Performs other duties as assigned.

Qualifications Summary 

    • Ambulatory, public, community, mental health, emergency, SUD, complex care coordination, case management or critical care nursing experience is preferred.
    • Familiarity with diverse or marginalized communities are highly desirable.
    • Bilingual (English/Spanish) preferred.
    • Education
    • Associate’s, Bachelor’s or Master’s degree in Nursing from an accredited college or university.
    • Current Registered Nurse licensure in the State of Colorado.
    • Basic Life Support certification.
$31.19 - $37.42 an hour
