In October 2021, we launched our new ‘Food Club & Opportunity Hub’ to expand efficient, impactful healthy food assistance with the on-site integration of other pathway programs. More recently, we announced a partnership with Dwelling Place to help address the affordable housing crisis here in Holland. In July 2023, we publicly launched our expansion of Lakeshore Food Rescue (www.lakeshorefoodrescue.org).

Position Details
Position Type: Full-time, Regular (32-40 hours/week)
Overtime Status: Non-Exempt
Reports To: Director of Food Access Programs

Department: Food Access (Food Club, HEAL)
Primary Location: Food Club & Opportunity Hub
Position Term: This position is funded via a grant through September, 2025. We anticipate the likely extension of this position’s work based on success during this term. 
Compensation & Benefits: $27 - $30.50 hourly, eligible for full-time benefits

Summary
The Healthcare Partnerships Manager will work to build partnerships between Food Club’s health food access programming and local healthcare providers to explore the work of partnering together to address social health needs in our community. This role will provide leadership and support in the achievement of Action House’s strategic efforts to optimize health outcomes for Food Club members with nutritionally relevant health conditions. This role will be responsible for building relationships with local healthcare providers, developing new referral pathways, and creating a framework to track and share individual and community health outcomes and shared wins. 

An ideal candidate will have experience working within or in collaboration with the healthcare sector, be comfortable navigating the healthcare system, enjoy building mutually beneficial relationships and referral pathways, and have the ability to think strategically and creatively to measure and track shared wins and long-term health outcomes.  The Healthcare Partnerships Manager will have the opportunity to work independently and take initiative on new ideas within the program.

Responsibilities & Essential Duties

    • Work closely with program leadership to shape and implement organizational vision and approach for healthcare-food access partnerships tied to the organization’s Strategic Vision & Pillars. 
    • Responsible for identifying, researching sourcing and maintaining strategic healthcare provider partnerships, and developing a strategic outreach plan to map out both current and future partnership opportunities. 
    • Build, maintain and grow a process-driven detailed approach to healthcare provider partnerships and relationship management.
    • Manage provider relationships with an emphasis on collaboration and shared wins, working to achieve the maximum partnership benefit for both parties.
    • Develop and implement a streamlined referral process based on relevant indicators to provide an effective pathway for patients to enter into Action House services.
    • Assemble and maintain relationships with strategic community partners and advocates, actively working to sharpen approach, develop and shape desired program outcome, and robust evaluation plan. 
    • Develop and maintain an outcome and data tracking system to show program and grant impact to key stakeholders. 
    • Ensure consistent, high quality project management and strategy, ensuring reporting and milestone deliverable deadlines are met. 
    • Collaborate with leadership and other teams to ensure flow of information and success of key project outcomes.
    • Research and stay abreast of all relevant local and national programming interventions and best practices in the sector.
    • Partner with the Healthy at Home Coordinator and healthcare partners to develop and implement medically-tailored meal kits that strategically target diet-related health conditions. 
    • Other duties as assigned.

Supervision Received

    • General Direction: Receives broad guidance and direction and has some autonomy in decision-making and flexibility in determining how to achieve goals. Supervisor provides support and resources as needed but may not be directly involved in day-to-day decision-making or task assignments.

Education & Experience

    • 4+ years of relevant experience required 
    • Some college required. Bachelor's Degree preferred.; or in place of a degree, equivalent years of experience

Qualifications

    • Previous experience or knowledge of working in a healthcare setting, healthy eating programming, food as medicine, or in social drivers of health.
    • Strong demonstrated partnership-building experience with the ability to engage a wide range of internal and external stakeholders, including healthcare providers, peer organizations, guests and volunteers. 
    • Proven ability to learn, think, plan, act and creatively problem-solve, with limited backstopping. 
    • Highly collaborative with the ability to take initiative, prioritize, and manage multiple projects at once. 
    • Previous experience preferred in leading performance and outcome based initiatives, including evaluation and reporting.
