CORE (Community Organized Relief Effort) is a nonprofit organization whose mission is to build sustained, community-based response programs to natural disasters. In response to the COVID-19 pandemic, CORE GA provides free vaccinations and additional resources to disadvantaged communities and emerging hotspots around Georgia. This is accomplished through maintaining strategic partnerships with local health agencies and community-based organizations to reach high-risk populations in vulnerable areas. The Health Outreach Coordinator will be responsible for outreach and education to organizations and individuals throughout communities served. Additionally, the Health Outreach Coordinator will lead the team’s benefits navigation, resource coordination and incentive distribution activities. 

Responsibilities:

    • Understand and effectively communicate CORE’s mission, vision, core values & strategic direction to external groups  
    • Conduct intake, follow up, refer, and assist clients to community resources and public assistance programs.  
    • Assist clients with screenings and applications to public assistance programs such as SNAP, WIC, TANF, CAPS, Medicaid, etc. 
    • Develop trust and rapport with clients and refer to additional programs and resources as needed   
    • Maintain and share community resources information with CORE GA program staff and clients  
    • Track and monitor program data and deliverables   
    • Identify and develop relationships with resource organizations in designated Health District who can participate in/lead response activities 
    • Engage with formal and informal community leaders and influencers, including churches, local businesses, neighborhood associations, and other civic organizations  
    • Work closely with the Team Supervisor, Assistant Team Supervisor and Community Health Worker to provide a continuum of services   
    • Adhere to all program guidelines, policies, protocols and workflows 
    • Report to Operations Manager and Team Supervisor for direction and communicate significant incidents 
    • Other duties as assigned

Qualifications:

    • Minimum 1-2 years’ experience in social work or public health or other fields providing community resources and transformation 
    • Available Full-Time, 40 hours a week, including weekends (weekly schedule varies, 5 days/week) 
    • Live in, or near the local health district office 
    • Fluency in another language, particularly Spanish is preferred  
    • Access to reliable transportation to travel throughout assigned Health Districts 
    • Comfortable traveling up to 1-3 hours from work base in personal vehicle  
    • Comfortable working outdoors throughout the year 

Experience:

    • Demonstrated experience working with underserved/under resources communities 
    • Demonstrated experience in identifying and developing partnerships with organizations to meet needs of low-income clients

Skills and Competencies:

    • Excellent analytical skills, observation and analysis capacity  
    • Very good knowledge of G-Suite and Microsoft Office (including MS Word, Excel) 
    • Readiness to commit and adhere to the values, mission and vision of CORE 
    • Excellent writing, reporting and communication skills 
    • Proficient in MS Office applications and referral platforms   
    • Close attention to detail and a high level of accuracy   
    • Flexibility to adapt to changing priorities and needs of the organization and adaptability in work style and work schedule, including ability to work for extended days and on weekends during critical points in the program 
$25 - $25 an hour
In accordance with law, the expected pay range for this position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.
