JOB SUMMARY: Under the direction of the Nurse Partner (RN/LPN) and Team Leader (RN Nurse Manager), the Certified Nursing Assistant (CNA) provides direct care and services to support our residents’ daily needs and overall well-being.  While this care is provided under the guidance of the nursing team, the Care Partner is the residents’ closest advocate in the Household.  All Care Partner tasks are performed in accordance with current standards, guidelines, and regulations, and in a safe and efficient manner. Personally responsible and held accountable for following FellowshipLIFE’s mission, values, and Fellowship Spirit culture, while maintaining and living out our philosophy of person-directed care.

Core Functions

    • Attends to all resident personal hygiene and care needs, including but not limited to grooming, dining and nutrition, mobility, elimination, psychosocial and safety needs. 
    • Supports residents in self-care and provides assistance as necessary for those unable to provide for their own needs.
    • Assist in the dining room during all meals.  Provide tray service to residents who may be unable to attend the dining room.
    • Assists with adaptive equipment/techniques and special care needs within the scope of training. 
    • Assist and safely transfer residents as needed from bed to chair as well as assisting with ambulation as required.
    • Weigh residents on admission and then daily, weekly and monthly as requested by the nurse or indicated in care plan.
    • Assists with the comfort and care of residents with special considerations for the very ill or dying residents; ensures a private and calm environment for the family; provides supportive care to the roommate; provides post-mortem care with dignity and respect.
    • Reports changes in condition or function and unusual incidents or accidents to nurse; documents as necessary.
    • Receives report from previous shift; provides report to oncoming shift.
    • Assures that all work areas, including resident suites, are kept clean, free of clutter, and are adequately stocked.
    • Distribute linens and towels.  Make up and turn down bed daily and ensure linens are clean and dry.
    • Supports the Household housekeeper in maintaining a clean, safe environment and in caring for the residents’ personal laundry.
    • Supports all aspects of the Household environment, including pets, plants, and guests, facilitating all planned and spontaneous needs and activities.
    • Follows universal precautions, proper infection control, sanitation, and safety standards of practice in all work and activities.
    • Responds quickly and empathically to all residents’ requests for assistance and care.
    • Observes residents closely, identifying changing needs and conditions at first indication, and reports immediately to nursing or neighborhood leadership as appropriate
    • Works with all Household team members, led by resident and family preference and Daily Pleasures to meet the needs and desires of all residents
    • Continually seeks opportunities to meet residents’ psychosocial needs through both care planned and spontaneous actions

Essential Functions

    • Person-Directed, Quality-Focused: Care Partners must develop an innate ability to observe and connect with residents in a way that gives them clear insights into each individual’s Daily Pleasures (preferences) and concerns. Care Partners must demonstrate the intention to strive for high efficiency while always respecting and preserving residents’ individuality. Ensure that Resident Rights are always respected.  This includes but is not limited to self-determination, fair and equitable treatment, freedom from abuse or neglect and the right to wage complaints. Maintains confidentiality by adhering to the HIPPA policy. 
    • Community and Team Builder: Care Partners must have strong team skills, both in terms of participating in team decisions and completing tasks in a team format. They should be skilled at facilitating residents’ involvement in community meetings. Assumes the role of coach for new team members 
    • Conflict Resolution: Care Partners should have a good understanding of healthy conflict resolution techniques and demonstrate a willingness to work out any personal conflicts with others in a way that is in keeping with the Household culture. Work with the Nurse Partner/Leader to find plausible solutions/responses that are resident-centered and appropriate.
    • Interpersonal Sensitivity and Communication Skills: Care Partners should have a strong ability to communicate with residents clearly and respectfully, without patronizing or manipulating. Care Partners should be sensitive to the issues and difficulties that frail elders may face, both physically and emotionally. Care Partners should be able to clearly give feedback and ask for information from other team members and family members as necessary. Courteous and cooperative interaction with guests, family members, and team members.
    • Dementia Sensitivity: Demonstrates an ability to facilitate the ability of persons living with dementia to enjoy daily life of the Household in meaningful ways. Demonstrates an understanding of – and intention to avoid – care giving practices that detract from a person’s sense of personhood. Is sensitive to the challenges a person living with dementia faces in terms of communicating their needs and Daily Pleasures, as well as maintaining a sense of personhood, in the face of degrading cognitive abilities.
    • Organizational and Legal Compliance: Abides by the current laws and organizational standards and procedures designed and implemented to promote an environment which is free of sexual harassment and other forms of illegal discriminatory behavior in the workplace. Abides by and supports the organization’s code of conduct by exhibiting behavior that is set forth in the code of conduct.  Performs duties in accordance with the code of conduct and completes annual compliance and educational training.

Additional Responsibilities

    • Responds to call bells promptly (within 5 minutes) for all residents. Enlists fellow team members to ensure safe completion of assigned duties.
    • Identifies self and purpose when entering resident’s suite; addresses residents by their full names, unless specified.
    • Maintains a safe, comfortable, and functional work environment by assessing/reporting potential hazards, and supporting continuous quality improvement.
    • Reports all incidents/accidents to self, resident, or others immediately.
    • Actively participate in being part of a learning and teaching organization.  Willingness to continue to learn and seek improvement.  Attend in-service training programs.
    • Stay informed of and adapt to changes in the senior living industry, as well as company policies, procedures, and best practices.
    • Other duties as assigned: There are times based upon unusual work volume (increase or decrease), emergencies, or severe weather conditions, when it will become necessary for you to accept and perform a work assignment or assist with tasks which are outside of your regular duties or cross-trained assignment in which you possess the basic knowledge and skill to perform the assignment safely.

Minimum Acceptable Qualifications

    • Current and valid/unencumbered Certified Nursing Assistant (CNA) certification.
    • High school graduate or GED preferred but not required.
    • Strong interpersonal skills are necessary to communicate and respond effectively to the instructions and requests from a diverse group of residents, visitors, and staff, and to provide information with courtesy and tact.
    • Clear written and oral communication skills in English in person and over the telephone.
    • Must possess a strong work ethic. Must be dependable and have a clear understanding of the need for punctuality and limited absences, ensuring all members of the Household are fully supported in their daily activities.
    • Must demonstrate willingness and availability to work in the event of callouts or other emergency situations (i.e. weather or other natural disaster) where overtime may be required. Must adhere to schedule requirements, including scheduled weekends and holidays. Able to accept changes in work assignments to meet residents’ and scheduling needs.
    • Ability to prioritize tasks and complete them with autonomy and accountability. Accurate, detailed, and dependable, with a commitment to delivering excellent care.
    • Other duties as assigned. There are times, based upon unusual work volume (increase or decrease), emergencies, or severe weather conditions, when it might be necessary to accept and perform a work assignment or assist with tasks that are outside of your regular duties or are cross-trained for an assignment in which you possess the basic knowledge and skill to perform the assignment safely.
