Position: RN MDS Coordinator

Key Qualifications:
Active RN license in the State of New Jersey2–5 years of experience as an MDS CoordinatorStrong attention to detail and organizational skillsExcellent written and verbal communication abilitiesPassion for delivering high-quality, person-centered careConduct comprehensive assessments of residents to develop and implement individualized care plans in accordance with MDS guidelines.Ensure accurate documentation of patient care and maintain compliance with HIPAA regulations.Collaborate with interdisciplinary teams to facilitate effective communication and discharge planning.Utilize EHR systems to manage medical records and track patient progress.Provide education and support to patients and families regarding care options and resources.
