In-Field RN at Florence
Florence’s field nurse is responsible for delivering in-home and in-hospital visits for Florence's primary care patients. The In-Field RN exclusively serves Florence's primary care patients, and you will always be visiting and supporting patients who have existing, trusted relationships with Florence's clinic (unlike many home health agency roles, in which the nursing team is venturing into the homes and other settings of new patients with no existing relationship with the team).

Florence's field team members are an out-of-clinic extension of our in-clinic primary care team, enabling us to support our patients in their homes when needed for welfare checks or medical support, as well as visit our patients in the hospital and post-acute settings whenever they are admitted -- to offer a friendly face in hard times, see if there's any additional support or care we can provide them during their stay, and ensure the discharge process is well prepared and they have a strong recovery once they return home.

While primarily an in-the-field, mobile position, the Field RN also has space at our primary care clinic where you can spend time working on charts and recharging with the team when not at home or hospital visits.

Responsibilities of the RN (in-field) include:
- Visiting our primary care patients at their homes and in the hospital and in other post-acute environments, to deliver care, support, and check-ins as a mobile extension of the primary care team.

- Working together with the primary care provider and the rest of the staff as one, through Florence's electronic health record and other team communication tools.

- Detailed, smart, and compassionate preparation and ownership of post-hospital discharge planning, coordination, and follow-through, to make sure nothing falls through the cracks, tailored to each patient's needs.

- Experience delivering wound care, both routine and more high-need

- Experience administering IV therapy, both routine and more high-need.

- Thorough patient chart review and information gathering/preparation to ensure the best use of time with each patient during their in-hospital and in-home visits.

- Tele-based patient triage as needed.

- Other duties as assigned.

$115,000 - $130,000 a year
