We have an immediate need for a full-time Home Visit Specialist to join our Care Team.
Our Face Visit Specialists work with our clients to provide information about the program that we work with, providing a personalized experience for each individual. They work closely with a care team to ensure that the client's needs are met in relation to their medical conditions while collaborating with providers offices throughout the community focusing on each client's plan of care. Additionally, they plan periodic visits to client's homes to update files for compliance purposes. The main goal is to ensure that we have genuine relationships with our clients and that they feel loved and special.
Applicant must have excellent communication skills, have the ability to work cohesively within a team setting, be accountable, be a self starter, and must have reliable transportation (mileage is reimbursable).

$21 - $21 an hour
Eligible for a $5,000 sign on bonus paid as follows
$2500 after 90 day probationary period
$2500 after completing 6 months with the company
