We are seeking a highly skilled and compassionate Resource Nurse in the Farmington, NM area to join our team. This role allows you to stay close to home while gaining diverse experience, making a meaningful impact on patients' lives and contributing to different communities within the state.
This role is a Full-Time benefit eligible opportunity, with care being provided at the patient’s residence. Patients range from high acuity to those who only need basic medical management.
Once our patients are accepted into the program, there is no expiration for this benefit, which allows our team of caregivers to build lasting relationships with their patients for years to come.
You’ll get to advocate for the patients and work one-on-one to provide them with the care that they need.
This role will collaborate with CNA’s, HHA’s, Case Managers, and the patient’s physicians.

Requirements:
- RN or LPN License – Active and valid in the state of practice (Compact license preferred, but not required).
- Experience in Home Healthcare – Comfortable providing care in patients' homes on a daily and weekly basis.
- Willingness to Travel to Patient Homes – Ability to travel to various patient homes, including remote or rural areas in Northern New Mexico.
- Familiarity with Reservation Terrain – Knowledge of local geography and the challenges of the reservation area.
- Flexibility for PRN Shifts – Availability for on-demand, per diem shifts, including evenings and weekends.
- Strong Clinical Skills – Ability to manage patient care in a home setting, with effective assessment and interventions.
Preferences:
- Experience with Diverse Patient Populations – Ability to handle a variety of medical conditions and needs.
- Ability to Work Independently – Comfortable managing patient care without direct supervision in home settings.
- Comfortable Navigating Remote Areas – Ability to navigate less-developed or difficult-to-access areas.
- Strong Organizational Skills – Ability to handle multiple patients and appointments efficiently.
- Cultural Sensitivity – Understanding and respect for the cultural practices and needs of reservation communities.
Job Responsibilities:
- Execute physician prescribed plans of care.
- Complete compliant documentation of the care provided in the system of record.
- Manage and administer prescribed medication, treatment, and therapies.
- Conduct patient assessments, coordination of care.
- Perform various activities associated with daily living.
- Monitor vitals and GI intake and output.
- Assess skin integrity and administer wound care.
- Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression.
- Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients.
- Provide education and training to the patient, their family, and/or the Home Health Aide.
- Work with patients to be done one-on-one in the patients home to provide for a comfortable environment.
- Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants.
- Perform other personal care services as necessary to meet the patient’s needs.

Schedule:
- Ideally it would be a Monday - Friday, 8-5 schedule, but will require flexibility to meet the demands of our patients.
$83,871 - $104,839 a year
Quarterly Bonus opportunities up to 4k per quarter.
