NKS Health Mississauga is looking for a qualified Full-Time Registered Practical Nurse (RPN) to join our team!
As part of this role, you will be responsible for both coordinating LTC home clinic visits and for visiting LTC homes in Ontario (primarily in the GTA area) to support physicians and assist with providing care to our patients. This is a Full-Time position that is a hybrid between in-clinic visits and in-office coordination and requires you to have a valid driver's license to meet the travelling requirements of the job. You will be reimbursed for travelling expenses, as per our business expenditure policy. As an RPN with NKS Health, you will be responsible for providing exceptional patient care to enrich the lives of elderly patients daily.

Responsibilities

    • Enrollment of new LTC homes to the program
    • Enrollment of new patients from LTC homes to the program
    • Coordination with LTC homes, specialists, and pharmacies to schedule clinic visits.
    • Scheduling clinic days within CRM system.
    • Coordination with power of attorney (PoA) prior to provide consent prior to treatment of patients.
    • Proper organization and follow ups to ensure all necessary pre-clinic procedures are completed prior to clinic days.
    • On clinic days, connect with LTC support person to let them know you have arrived.
    • Locate the medication.
    • Confirm all patients on the list are still at the home.
    • Cross reference names with medication received to ensure accuracy.
    • Attend floors for patients on schedule to ensure they are in their room/bed.
    • New referrals, ensure MARS print outs are available for physician.
    • New referrals, ensure you have POA contact info for physician to call for consent or discussion re: assessment.
    • Ensure all special notes are communicated to physician ie: INR reports, family requesting calls, family requests for physician regarding care etc.
    • If patients aren’t in their room, locate PSW or staff to have person return to their room and in bed.
    • Remove outer layer of clothing so physician can assess.
    • Assist in positioning if physician requests.
    • Prepare and reconstitute medication for injections by adhering to aseptic technique
    • Document any changes physician requests during assessment and complete clinic day reports
    • Document follow up for families who require same.
    • Put clothing back on patient when done.
    • Ensure physician has access for charting (sometimes in PCC)
    • Ensure all notes are detailed and accurate for follow up to physician, pharma and LTC.
    • Documentation of clinic day reports on patient files within CRM
    • Reporting any adverse events if they occur via CRM

Qualifications

    • Valid Standard First Aid and Level C CPR (required)
    • Registered Practical Nurse certification (required)
    • Ability to stand, bend, walk, crouch, kneel and reach freely and for extended periods
    • Ability to lift 23 kilograms independently; ability to assist clients with standing, walking, sitting; able to reposition clients in bed
    • Demonstrated ability to provide exceptional personal care and customer service
    • Valid Vulnerable Sector Criminal Check
    • Updated immunization record (TB test within 1 year; Covid-19 Immunization)
    • Proficient verbal and written English communication skills
    • Experience with reconstituting and preparing medication using aseptic technique would be an asset
    • Valid G level driver’s license and access to a motor vehicle

Minimum Skills, Knowledge and Ability Requirements:

    • Superior customer service skills
    • Demonstrated teamwork abilities
    • Organizational skills and experience with documentation on patient files within CRM
    • Ability to manage multiple tasks and priorities at once
    • Comprehension of medical terminology, Ability to read and interpret patient charts
    • Strong analytical skills including interpretation of regulation and legislation.
    • Ability to make decisions using sound judgment
    • Ability to work autonomously
    • Strong organizational skills, creative problem solving and results oriented.
    • Strong time and project management skills and sense of urgency
    • Ability to analyze current enrollment volume by MD
    • Ability to analyze enrollment potential based on prescription volume of other biologics
    • Ability to engage with a physician and his/her staff as a trusted and well-informed medical counterpart
    • Must have a valid driver’s license and possess a car
    • Ability to conduct Continuous Healthcare Education (CHE) events with HCPs, which may occur outside regular business hours
    • Can interact with multiple sales reps and district managers from the commercial team
    • Superior verbal and written communication skills, interpersonal skills and
    • Attention to detail.
    • Ability to speak multiple languages an asset
    • Established relationships with Customers in a given territory, an asset
    • Knowledge of public, private and federal reimbursement an asset
    • Hold and maintain a valid, non-expired, unrestricted driver’s license
