As a Care Champion at Health at Scale, you will work with an exceptional team of clinicians including physicians, nurse practitioners, and nurses to provide virtual outreach to our clients’ employees and members with complex healthcare challenges.
Members who need care may be unaware that they have gaps in care or risk factors that need to be addressed. Using AI-powered proactive identification and outreach, Care Champions engage the members who need help at early stages.
Care Champions will empower members with personalized health education content relevant to their unique conditions, helping to improve health literacy. In addition, Care Champions will equip members with hyper-personalized provider matches (PCPs, specialists, procedures and imaging) that account for the thousands of characteristics that make members unique.
Responsibilities
- Directly engage members through various channels to help them navigate through their complex healthcare challenges
- Guide the design and deployment/workflow integration of clinical user interfaces and analytical pipelines
- Structure and coordinate trials to demonstrate company software and services
Requirements
- BSN or MSN (Nurse Practitioner) with current and active RN or NP license
- 3+ years of experience in clinical practice
- Experience with clinical data, insurance claims and billing, data coding
- Experience with care delivery, gaps in care, utilization review
- Excellent communication, interpersonal and organizational skills
- Fluency in both English and Spanish
