Care Coordinator Questionnaire

  • Do you have at least 1 year of experience in an administrative, operations, or customer service role?
  • Do you have experience in social services, care coordination, or healthcare navigation? If so, please describe the populations or systems you supported and share an example of how you’ve researched and evaluated community or Health Plan resources. What criteria did you use to determine quality, accessibility, cultural fit, or insurance compatibility?
  • Are you comfortable managing inbound phone calls, voicemails, and shared inboxes—and routing messages or tasks to the appropriate team members?
  • Are you available to work on a part-time basis (up to 20 hrs a week) at least 4 days per week between the hours of 8am–8pm EST?
