We're looking for an amazing Care Advisor to join our Care Team in the San Francisco Bay Area. We work in a fast-paced, team-driven environment that allows us to deliver the best care to our seniors and their families. As a Care Advisor with Honor, your role will be to assess potential care recipients and provide guidance to help determine their eligibility for Honor non-medical home care services. Our Care Advisors provide support to our care recipients by being great listeners and understanding client needs so the recipient can receive the best personalized care Honor can provide.

Responsibilities

    • Travel throughout the local region (entire Bay Area) — must have reliable transportation
    • Must be available to work two weekends per month
    • Visit clients in their homes and facilities
    • Represent Honor in face to face contact with potential care recipients
    • Gain deep knowledge of Honor services and policies
    • Take a holistic view of patient journey
    • Assess client's activities of daily living and advise client on best program for care
    • Evaluate client's home environment for safety
    • Develop a clear care plan with the care recipient and their care manager (family member, friend, etc.)
    • Complete online electronic forms and applications related to client's health status, diagnoses, medications and other pertinent factors — must be comfortable using tablet and cell phone
    • Coordinate with client care team post assessment to coordinate care
    • Participate in proactive service recovery visits


Skills & Experience

    • Prior experience with assessments or patient intake
    • Working knowledge of chronic health condition terminology
    • Experience building rapport in a short time frame
    • Understanding of non-medical home care
    • Ability to operate independently and use discretion
    • Strong work ethic and commitment to success
    • Caring, patient, empathetic demeanor
    • Agile and flexible team member
    • Prior sales experience a plus, but not required
