As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.
Schedule: Monday- Friday with flexible scheduling, 32 patient contact hours minimum. Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone On-Call
EMR: Athena
On-Call: Phone call only, in a rotation every 7-8 weeks for one (1) week at a time. Average call volume - <1 call per WEEK (as of 8.1.24). 24/7/365 physician back up.
Requirements:
- Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB)) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA).
- Unrestricted DEA licensure and prescribe authority (per state practice regulation).
- At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred. If no formal training in weight management, willingness to participate in knownwell clinical education and training plan for weight management and/or pursue OMA APC certification. - You have an active, unrestricted license to practice in Massachusetts, or willingness to obtain. Willingness to obtain additional licensures if needed based upon business needs.
- You have excellent communication and interpersonal skills.
- You’re dedicated to staying current with the latest developments in primary care and obesity medicine.

- You believe in shared decision making with a patient. You honor the goals a patient has for their health, and you partner with them to help them achieve those goals.
- You have high emotional intelligence and know how to make a patient feel at ease – without judgment and without shame.
- You’re willing and excited to try new technologies that are both patient and provider oriented.
- You are excited to work with a collaborative team of physicians, APC’s, registered dietitians, medical assistants, health coaches, and other staff.
- We are unable to sponsor visas currently, so you are authorized to work in the United States.
Responsibilities:
- Practice exceptional medicine within knownwell
- Sees adults for primary care, often with a focus on metabolic health, or adults for metabolic health only. Share a panel with MD/DO + other APCs.
- Embodies the knownwell mission and values of patient inclusivity in how you deliver care.
- Coordinates with the broader care team to ensure the patient has the support they need.
- Documents as necessary to ensure accurate billing, including review of remote patient monitoring data.
- Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response.
- See adolescents ages 12+ for metabolic health and primary care.
- Ensure panel is open to any individual, regardless of their current health status and comorbidities.
- Listen to the aspirations of the patient and ensure care is tailored to the patient’s unique health journey. Provide an environment in which patients feel comfortable and free from judgment.
- Offer exceptional encouragement as patients reach their goals.
- Coordinate across the collaborative care team
- Escalate any concerns to your dedicated site leadership team.
- Provide clinical guidance to medical assistants and nursing teams as assigned.
- Stay current with licensure and appropriate re-certifications.
