LifeStance Health is hiring a full-time licensed therapist (social workers, counselors, marriage and family therapists) with excellent communication skills and clinical acumen to join our Integrated Behavioral Health (IBH) team!
 
This position is embedded in a Family Medicine clinic and the clinician will serve as a core member of the Collaborative Care team as well as the larger medical team. The Collaborative Care Clinician (CCC) will be responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s Primary Care Provider (PCP) and, when appropriate, other mental health professionals. Ideal candidates have experience working with patients across the lifespan (ages 12+) using empirically supported treatment, working in a primary care setting, and who enjoy working in a fast-paced environment. 
$110,000 - $130,000 a year
Position highlights are as follows:
        Serve as a key member of the Collaborative Care team, which consists of the PCP and a psychiatric consultant. The CCC will support the mental and physical health care of patients, closely coordinate care with the patient’s PCP, and other mental health providers where appropriate.
        Have a clearly distinguished role from specialty mental health therapist practice model.
        Work with patients on presenting concerns across the lifespan (e.g., anxiety, depression, ADHD, adjustment to/management of chronic health conditions) utilizing brief, empirically supported treatment modalities including motivational interviewing, behavioral activation, problem-solving treatment, among others.
        Employ a population-based practice management model with flexibility to be interrupted to respond to urgent/emergency situations and “curbside consults”.
        Screen/assess patients for common mental health conditions, facilitate patient engagement and follow up care with the support of a dedicated behavioral care coordinator.
        Participate in regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s PCP.
        Track patient engagement and clinical outcomes using a registry.
        In consultation with the patient, PCP, and psychiatric consultant, facilitate treatment plan changes for patients who are not improving as expected and who may need more intensive or more specialized mental health care.
        Day shift with no call coverage.
        Dedicated administrative and care coordination support from LifeStance staff.
