THE OPPORTUNITY:
The Clinical Pharmacist will serve as both a patient-facing provider and a clinical resource to MHN’s care teams. This role will focus on medication management, patient education, and chronic disease support, working directly with patients and families to optimize medication use and improve health outcomes. By embedding pharmacy expertise into MHN’s partner practices, the Clinical Pharmacist will play a vital role in advancing medication safety, increasing adherence to guideline directed therapy, reducing hospital admissions and readmissions, and improving quality performance in value-based care models including Medicare Advantage Stars performance. This is a part-time opportunity, approximately 16 hours per week.

WHAT YOU CAN LOOK FORWARD TO:


    • Direct Patient Care 
    • Provide one-on-one counseling to patients and caregivers on medication use, adherence, side effects, and cost-effective options. 
    • Conduct comprehensive medication reviews and reconciliation, particularly for patients with chronic conditions, multiple medications, or recent hospitalizations. 
    • Partner with care managers, health center pharmacists, and primary care providers to support patients in meeting treatment goals, including diabetes, hypertension, COPD, and other chronic conditions. 
    • Identify and address barriers to adherence, including social determinants of health (e.g., affordability, access, health literacy). 
    • Support transitions of care by ensuring patients understand their discharge medications and follow-up care plan. 

    • Clinical & Team Support 
    • Work collaboratively with physicians, nurse practitioners, pharmacists, care coordinators, and behavioral health providers to develop patient-centered care plans. 
    • Provide drug information, guideline updates, and recommendations for therapy adjustments or deprescribing. 
    • Educate care teams on safe prescribing practices, formulary optimization, and cost-saving strategies. 
    • Participate in case conferences and multidisciplinary team huddles at FQHCs. 

    • Population Health & Quality Initiatives 
    • Use clinical and claims data to identify patients at high risk for medication-related problems. 
    • Support value-based performance initiatives by developing and implementing interventions to improve medication adherence and close quality gaps (e.g., HEDIS, Star measures). 
    • Contribute to MHN’s quality improvement projects, care pathways, and clinical protocols related to medication management. 
    • Represent MHN in external forums with payors, providers, and community partners as a pharmacy subject matter expert. 

WHAT YOU’LL NEED TO SUCCEED:

    • Doctor of Pharmacy (PharmD) degree required. 
    • Active pharmacist license in the State of OK and IL preferred, or ability to obtain a license in these states. 
    • Minimum 3 years of clinical pharmacy experience, preferably in ambulatory care, primary care, or population health. 
    • Direct patient care experience required; background in FQHCs or working with underserved populations strongly preferred. 
    • Familiarity with value-based care, chronic disease management, and quality measurement programs. 
    • Strong communication skills with the ability to counsel patients, collaborate with care teams, and build provider relationships. 
    • Experience with EMRs, care management tools, and interpreting clinical/claims data preferred.
