Your responsibilities will include evaluating patients' home environments and functional status, as well as contributing to the development of comprehensive care plans.

Requirements:
- Licensed to practice as a physical therapist in the state.
- Doctorate or Master's (MS) degree in physical therapy.
- Minimum of one (1) year of experience as a physical therapist.
- Previous home health experience is not required, but welcomed.
