Registered Nurse - Home Health

This is a full time position, working Monday-Friday 8:00am-5:00pm PST

Compensation Band: $50.00 to $60.00 per hour

The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience.  The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Key Responsibilities:

    • Performs initial and ongoing assessments to identify patient and family needs, including physical evaluations and medical history.
    • Develops, updates, and manages individualized care plans based on nursing diagnoses, patient conditions, and changing needs.
    • Provides direct care such as administering medications, treatments, and nursing procedures as prescribed.
    • Educates and counsels patients and families on health conditions, care routines, and discharge planning.
    • Maintains accurate clinical documentation and communicates regularly with physicians, especially regarding changes in patient status or care needs.
    • Coordinates with physicians and community health providers to ensure cohesive care delivery.
    • Participates in on-call rotation and ensures needed equipment and services are in place.
    • Supervises and evaluates home health aide care every 14 days.

Position Qualifications:

    • Graduate of an accredited school of nursing. 
    • One (1) year of recent acute care experience in an institutional setting preferred or one (1) year of recent experience in home care. 
    • Current licensure in State, BLS CPR certification.
    • Valid driver’s license and auto insurance. 
$50 - $60 an hour
