Pay range (depending on experience): $90-120k per year

Schedule/Shift: Full-time, M-F

Territory/Location: PNW region

Key Responsibilities:

    • Identifies and implements practices to improve quality and service.
    • Establishes processes for proactive identification of issues and tracking of corrective action to minimize negative impact and maximize learning
    • Ensures the agency's Performance Improvement Program is comprehensive and in compliance with all licensing and regulatory requirements consistent with policies, procedures, and standards.
    • Assists in the development, monitoring and presentation of internal quality measures and initiatives
    • Provides education and support to the clinical staff under the direction of the Director of Education in maintaining quality improvement outcomes and completion of the assessment questions to meet the requirement for HIS and OASIS assessments.
    • Participates in the development, implementation, and evaluation of best practices for Home Health & Hospice   
    • Responsible for the supervision of the Quality, Clinical Review, and Coding teams (Hospice)
    • Assists Clinical Managers in evaluating the quality of care provided by clinicians.  
    • Develop, implement, and collaborate on policies and procedures to ensure alignment with best practices and agency wide QAPI initiatives.
    • Educate and train individual branch QA staff, and collaborate with branch leadership on quality and process improvement
    • Facilitates learning experiences for staff through direct and indirect methods.
    • Additional duties as assigned.  

Qualifications:

    • Registered nurse in the State with current licensure to practice professional nursing.
    • Bachelor’s or master’s degree in nursing from an accredited program by the National League for Nursing preferred.
    • Recent management and clinical experience a must.
    • Demonstrated ability to supervise and direct professional personnel.
    • Working knowledge of HCHB tasks as it relates to quality and point of care documentation.
    • Excellent observation, verbal; and written communication skills.
    • Knowledge of State, CHAP, and Federal regulations in Hospice and/or Home Health.
