Speech-Language Pathologist (SLP) – Home Health
We are seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our home health team. The SLP is responsible for implementing standards of care in speech-language pathology and leveraging community resources to support individuals and families in achieving improved communication and swallowing outcomes in their homes and communities.
Key Responsibilities:
- Conduct comprehensive evaluations and assess home environments to support speech and language function.
- Develop individualized care plans based on identified speech-language deficits.
- Deliver evidence-based speech therapy using specialized techniques and/or equipment.
- Recommend communication aids or devices when appropriate.
- Collaborate with physicians and interdisciplinary team members to ensure coordinated, comprehensive care.
- Participate in case conferences, in-service training, and other meetings as required.
- Document treatment plans, progress notes, and outcomes in accordance with agency policy.
- Maintain clinical competency in current speech-language pathology practices.
- Identify and refer patients/families to other services when needs are outside the SLP scope.
- Educate patients, families, and caregivers on treatment protocols and strategies.
- Supervise home health aides when duties relate to the speech-language therapy plan of care.
- Perform additional duties as needed to support patient care and agency operations.
Qualifications:
- Master’s degree in Speech-Language Pathology from an accredited program, including internship, or as required by state regulations.
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA.
- Active state license to practice as a Speech-Language Pathologist.
- Minimum of one year of clinical experience in speech-language pathology.
- Strong verbal and written communication skills, with excellent organizational abilities.
- Current hands-on BLS (CPR) certification.
Travel Requirements:
This is a travel-based position. Candidates must have:
- A valid driver’s license
- Active auto insurance meeting state minimum requirements
- A reliable, operational vehicle
$65 - $75 an hour
