Join our team at Healthy Living at Home as a Home Health Physical Therapist Assistant (PTA) and play a pivotal role in delivering exceptional, comprehensive care to our patients. Position: Home Health Physical Therapist Assistant (PTA)
Location: Scottsdale, Phoenix, and surrounding areas
Pay: Competitive per-visit rate, negotiable based on experience
NVA Rate: $30/hour
Revisit Rate: $60/per visit
Responsibilities:
- Collaborate with our multidisciplinary team to provide personalized care plans for patients
- Assist in implementing rehabilitative programs to improve patients' mobility, strength, and function
- Conduct patient assessments and document progress accurately
- Educate patients and their families on at-home exercises and techniques for ongoing improvement
Requirements:
- Graduate of an accredited physical therapy assistant program with an Associate's degree in Science or Applied Science approved by the American Physical Therapy Association, Inc.
- Current certification or license as required by state law
- CPR certification
- Valid driver's license and access to a reliable, insured vehicle
