Licensed Vocational Nurse (LVN) - Home Health

This is a full-time position located in San Diego, CA

Coverage areas:

Compensation band: $35.00 to $40.00 per hour

The Licensed Vocational Nurse provides direct patient care under a registered nurse's supervision. Responsibilities include following the care plan, providing treatments, and working with the team to achieve positive patient outcomes.

Job Duties:

    • Provide direct patient care as per State Nurse Practice Act.
    • Follow the care plan set by the clinician.
    • Document care accurately and on time.
    • Educate patients and families about the diagnosis and care plan.
    • Coordinate home health services and report needs for other disciplines to the clinician or manager.
    • Use equipment and supplies efficiently.
    • Engage in personal and professional development.
    • Perform other assigned duties.
    • Be supervised by an RN monthly as per agency policy.

LVN Qualifications:

    • Graduate of an accredited practical nurse or vocational nursing program.
    • One (1) year nursing experience in home care, hospice care, institutional hospital or facility care preferred. 
    • Currently licensed as a licensed practical nurse or licensed vocational nurse in the State.
    • Complies with accepted professional standards and practice.
    • Demonstrates good verbal and written communication, and organization skills.
    • Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
    • Possess and maintains current BLS CPR Certification.
$35 - $40 an hour
