Our Tender Care Hospice team is looking to bring add an experienced RN Case Manager to the team!

End of life care is a specialized area of nursing that focuses on providing care and support to terminally ill patients and families. Our nurses play an important role in helping patients and families cope with the physical, emotional, social and spiritual aspects of dying.

Pay begins at $80,000/year. Our Compensation package includes benefits, mileage reimbursement and a Wellness Day just for you!

The Position

    • Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs.  Provides a complete physical assessment and history of current and previous illness (es).
    • Provides professional nursing care by utilizing all elements of nursing process.
    • Assesses and evaluates patient’s status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs and participating in revising the plan of care as necessary
    • Participates in on-call duties as defined by the on-call policy.
    • Ensures that arrangements for equipment and other necessary items and services are available.
    • Supervises ancillary personnel and delegates responsibilities when required.

Qualifications

    • Graduate of an accredited school of nursing.  One (1) to two (2) years of recent acute care experience in an institutional setting.
    • Good standing RN license in state of practice
    • BLS Hands-On CPR certification.
    • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
$80,000 - $85,000 a year
