Our Grover Beach team is looking for a passionate and experienced Registered Nurse to join our team. In this role, you'll have the opportunity to plan, organize, and direct home care services, with a strong focus on community health education and experience. You'll work alongside a supportive team of healthcare professionals, all committed to delivering personalized care and building lasting relationships with our patients and their families.

Job Duties/Knowledge
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluates patient nursing needs.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
Additional Duties
- Participates in on-call duties as defined by the on-call policy.
- Ensures that arrangements for equipment and other necessary items and services are available.
- Instructs, supervises and evaluates home health aide care provided every fourteen days.
Qualifications
- Graduate of an accredited school of nursing. One to two years of recent acute care experience in an institutional setting.
- Current licensure in State, BLS hands-on CPR certification.
- Valid driver’s license and auto insurance.
- Associate degree with one year of home health care experience preferred.
