Here at Mission, we provide top-quality in-home care services to home care patients. Our approach is simple: we work closely with our clients and their families to create a care plan that meets all of their needs, both physically and emotionally. We then match them with a care team that's the perfect fit! An important part of the success of this approach is our Medical Social Workers.

MSWs implement standards of social work care and help us address the needs of our patients and their families. Pay starting at $38 per hour.

Duties include:
1.       Assesses the psychosocial status of patients related to the patient’s illness and environment.
2.       Carries out social evaluations and plans intervention based on evaluation findings.
3.       Maintains clinical records on all patients referred to social work.
4.       Provides information and referral services for Mission patients and families/caregivers regarding practical and environmental needs.
5.       Provides information to patients or families/caregivers and community agencies.
6.       Serves as liaison between patients or families/caregivers and community agencies.


Position Qualifications
1.       Graduate of a Master’s program or Bachelor’s program with Master’s oversight in social work accredited by the Council on Social Work Education.
2.       Minimum of one year’s experience in health care.
3.       Experience in a home health care preferred.
4. Possesses and maintains current CPR certification.
5.       Possesses and maintains a valid driver's license, vehicle and auto insurance in accordance with Mission standards.
