Pay range (depending in experience): $80k - $100k/year

Schedule/Shift: Full-Time, Monday-Friday, 8AM-5PM

Territory/Location: Tempe, AZ

ACOS-C certification preferred; not required.

Key Responsibilities:

    • Collaborates effectively with clinicians, leadership, and other departments to ensure smooth workflow and communication.
    • Reviews and confirms documentation accuracy to support medical necessity, patient assessments, and care decisions (Admission/Recert/Discharge).
    • Identifies and resolves discrepancies in OASIS documentation, locking assessments in a timely manner.
    • Supports quality improvement initiatives in partnership with the Education Department, emphasizing OASIS accuracy.
    • Maintains up-to-date knowledge of home health regulations, CHAP standards, and SHP analytics to drive performance.
    • Ensures appropriate visit utilization and manages real-time alerts using SHP data.
    • Provides workflow coverage including holiday/weekend on-call rotation; completes all tasks before shift end.
    • Demonstrates strong organizational and problem-solving skills to prevent delays and support billing and reporting cycles.
    • Participates in case conferences, departmental meetings, and adheres to agency policies and procedures.

Qualifications:

    • Must be a graduate of an accredited School of Nursing
    • Current unencumbered license to practice as a Registered Nurse
    • Minimum of 1 year of home health experience with a working knowledge of Title XXII, Medicare Home Health Conditions of Participation, and OASIS
    • Excellent written and oral communication skills required.
    • Ability to handle stressful situations/deadlines.
    • Strong computer and software skills with knowledge of Home Care Home Base highly preferred.
    • Self-starter, goal directed with ability to work independently and stay focused without on-site supervision.
    • Ability to communicate effectively with agency staff, upper management, and other members of the health care team.
