Responsibilities

    • Plan, coordinate, and implement spiritual support activities that reflect the beliefs and values of the patient and family.
    • Perform a spiritual assessment within five days of admission to hospice
    • Attend interdisciplinary team meetings and collaborate with the team in developing the plan of care and ensure it reflect spiritual needs
    • Coordinate with or refer to local clergy as requested by the patient and family
    • Documents clearly, accurately and timely in the patient record
    • Follow agency policy & procedures and adheres to professional boundaries
    • Complies with HIPPA and confidentiality agreement
    • Assist families in planning funeral and memorial services when indicated
    • Collaborate with the bereavement coordinate to plan hospice memorial services for families of patients that have died
    • Attends interdisciplinary team meetings and is proactive in reporting current issues and collaborating with the team in developing interventions for the plan of care.
    • Coordinates care with other professionals to meet the emotional and psychological needs of the patient and family.
    • Conducts a risk assessment of complicating bereavement factors
$21 - $24 an hour
