Responsibilities
- Plan, coordinate, and implement spiritual support activities that reflect the beliefs and values of the patient and family.
- Perform a spiritual assessment within five days of admission to hospice
- Attend interdisciplinary team meetings and collaborate with the team in developing the plan of care and ensure it reflect spiritual needs
- Coordinate with or refer to local clergy as requested by the patient and family
- Documents clearly, accurately and timely in the patient record
- Follow agency policy & procedures and adheres to professional boundaries
- Complies with HIPPA and confidentiality agreement
- Assist families in planning funeral and memorial services when indicated
- Collaborate with the bereavement coordinate to plan hospice memorial services for families of patients that have died
- Attends interdisciplinary team meetings and is proactive in reporting current issues and collaborating with the team in developing interventions for the plan of care.
- Coordinates care with other professionals to meet the emotional and psychological needs of the patient and family.
- Conducts a risk assessment of complicating bereavement factors
$21 - $24 an hour
