Pay range (depending on experience): $55 - $60/hour + $5,000 Sign-On bonus!
Schedule/Shift: Full-time, Monday-Friday, 8AM-5PM
Territory/Location: Portland, OR
Patient Care Responsibilities:
- Conduct initial and ongoing assessments of patients and families to determine home health needs.
- Provide professional nursing care using the full nursing process.
- Assess and evaluate patient status by:
- Creating and initiating individualized care plans
- Reassessing needs regularly
- Updating plans as necessary
- Adapt care plans as the patient’s condition changes.
- Use health data to identify nursing diagnoses.
- Collaborate with patients and families to set care goals and develop plans.
- Deliver prescribed treatments and administer medications.
- Offer education and support to patients and families to address care needs.
$55 - $60 an hour
