The Role:

The Diabetes Nurse Practitioner (NP) will provide specialty level endocrine/diabetes care to a defined panel of patients using advanced Remote Patient Monitoring (RPM) technology. Applying clinical expertise and knowledge of diabetes management, the Diabetes NP will use decision-making skills to deliver remote diabetes care. At the initial consultation, the NP will formulate and then execute an individualized diabetes care plan based on current clinical standards and specific patient characteristics with the support of a lead Endocrinologist.  Once a treatment plan is initiated, the follow-up outreach, education and titration will be delivered collaboratively by a care team that the Diabetes NP will lead.  This care team is composed of Registered Nurse Care Managers (RNs), Certified Diabetes Care Managers and Education Specialists (CDCESs), and patient coordinators, all of whom will work simultaneously to move patients to their targeted health goals.  The Diabetes NP will serve as a clinical resource and educator for the team with backing from the lead Endocrinologist who oversees the overall program and provides full support to all team members.

Responsibilities:

    • Perform initial intake and consultation of new patients.
    • Formulate the diabetes treatment plan (in conjunction with an Endocrinologist when needed).
    • Execute the treatment plan: which includes setting A1C targets, ordering anti-diabetes medications/insulin therapies and transitioning to the care team for management.
    • Order self-monitoring devices such as blood glucose monitor, blood pressure monitor and continuous glucose monitoring systems.
    • Oversee clinical staff and care managers as they adjust dosages based on approved insulin and anti-diabetes medication titration protocols.
    • Modify the treatment plan as needed.
    • Serve as a resource to answer any clinical questions from the team and provide ongoing collaborative education to both patients and the health care team members. 
    • Assume co-management of the patients who are not achieving targets to reassess the treatment regimen, consider continuous glucose monitors, review barriers, and assess engagement of patients when needed. 
    • Engage the lead Endocrinologist for complex cases when needed.
    • Write brief clinical summary updates to the patients’ primary care providers at designated time intervals based on A1C trends and doctor preference.

Skills & Qualifications:

    • 5 years experience in diabetes care management required. 10 years of experience preferred
    • Certified and licensed Nurse Practitioner in California 
    • Bilingual (Spanish/English) required
    • CDCES or BC-ADM certification preferred 
    • Knowledge of diabetes technology including Dexcom sensors, Libre sensors/flash readers, diabetes smartphone apps, insulin calculators, insulin pump experience is bonus
    • Experience in formulating, initiating and modifying diabetes treatment plans
    • Knowledge and experience with ordering various insulins and other anti-diabetes therapies
    • Passion for improving patient outcomes long term
