We are seeking a passionate Intake Coordinator to join Patient's Choice Home Health Care a subsidiary of Nomi Health in Miami, Florida. You will report to the administrator or DON and assist the clinical team with staffing and the intake process. We provide skilled nursing, physical/occupational therapy, personal care and other home health services in Miami Dade and Monroe County, with the highest regard for quality of care and respect for our patients.

How you will make an impact

    • Receives referrals from hospitals and physician’s offices
    • Check referral portals and accept referrals. Communicate and discuss with DON
    • Coordinates home care services for patients who have been referred for services
    • Responsible for inputting patient demographic information into system
    • Verifies Medicare/Medicaid patient eligibility and obtain initial authorizations for services
    • Verifies primary care physician credentials
    • Assists with staffing and creation of schedules
    • Coordinates patient care with nurses, therapists, social workers and other members of the healthcare team
    • Establishes and maintains positive working relationships with current and potential referral sources
    • Ensures compliance with all state and federal regulations. Upholds patient and agency confidentiality

What we are looking for

    • High school diploma or equivalent
    • 1 + years’ experience in healthcare setting, preferably in home health, medical office, or related field
    • Must have basic clerical skills
    • Excellent verbal and written communication skills to interact with patients, families, healthcare professionals, and insurance representatives effectively
    • Strong organizational abilities to manage multiple tasks, maintain accurate records, and meet deadlines
    • Ability to empathize with patients and their families during challenging times and provide support with a compassionate approach
