We’re hiring LVN's or MA’s to run RPM for a cardiology practice in Torrance, CA. You’ll bring your experience educating patients on their chronic diseases such as hypertension and heart failure. You will join a network of nurses with flexible work schedules that will be available for part-time monitoring when the needs arise for different clinical practices.
Our Care Manager will be trained on the optimize.health online platform to onboard patients on their devices and monitor their vitals readings, as well as perform some patient outreach. You will also have the opportunity to play a role in guiding the development of further chronic care curriculum development. You will play an essential role in changing the lives of those to avoid severe chronic condition progression. This position does not require physical presence in the office. Preferred candidates should live in the Torrance area and are preferred to have availability to check in with practice as necessary.
Skills and Qualifications:
- 3+ years of experience in cardiology practice (preferred) or primary care practice
- Experience in remote monitoring or virtual care management program
- Computer literacy, comfortable learning and using digital health portal and video calls
- Full time, Mon - Fri
