Phoenix Paramedic Solutions is looking for a high-quality candidate for Valley Oaks Health brand new Crisis Stabilization Unit, The Haven. You will be working alongside the Valley Oaks Health Clinical Team and/or a Paramedic, Nurse, and/or EMT-B.

Hours:

    • 12-hour shifts, Day (8am-8pm) and Night (8pm-8am) shifts available  
    • 3 days on / 2 days off on a 2-week rotation 

Major Duties and Responsibilities:

    • Work with clients and our primary care and behavioral health clinicians to assess the level of intervention needed and address barriers to successful, long-term care.  
    • Provide crisis management, including but not limited to crisis assessment planning, individualized crisis safety plans, and de-escalation tactics.  
    • Understand and perform care escalation as needed.  
    • Provide follow-up with individuals in crisis within 14 days after contact is made and follow-up services for at least 90 days after contact.  
    • Establish professional, supportive relationships with individuals in crisis that includes rapport building, increased engagement, instillation of hope, care coordination, and referral.  
    • Must have the ability and willingness to accept direction and interact with law enforcement, hospital staff, and all other professionals in the utmost respectful manner. 
    • Keep equipment clean and in status-ready condition.  
    • Reporting of findings during treatment to appropriate facility staff in a timely manner 
    • Documentation of all calls you are assigned during a shift in a timely manner 
    • Knowledge of and performance of all Blood Borne Pathogen standards 
    • Cover On-call shifts on a regular weekly basis.   
    • Respond as part of the QRT team for Tippecanoe, Jasper, and White Counties 
    • Traditional Ambulance Transports.

Required to begin training:

    • High school diploma/GED.  
    • CRS / CHW Certification OR CAPRC Level 1 or Level 2 Certification 
    • AHA CPR Certification 
    • Some working knowledge of “Stages of Change” and “Motivational Interviewing” preferred 
    • Willing to provide services based on SAMHSA’s 10 Guiding Principles of Recovery  
    • Ability to pass a background check  
    • State Driver’s License 
    • Ability to pass vehicle insurance check 

Training Required On Job:

    • Drug screen Chain of Custody Company Certification. 
    • Portable Breath Testing Company Certification 
    • BBP Training (Annual)  
    • COWS assessment, DEA standards and policies, and pharmacology to include medications commonly used for MAT 
    • DOT Drug screen collector 
    • CPR, First Aid, BBP (BLS) 
    • CIT 
    • QPR 

Con-Ed Req:

    • Maintain Current CPR Certification (2 Hrs. bi-annually) 
    • BBP Yearly (1 Hr. Annually) 
    • Various Meetings discussing updates to services offered 
$16 - $18 an hour
Entrance base rate, based on experience:  

- $16/hr to $18/hr 
- $35,905 to $40,392 annually
