Planned Parenthood Los Angeles is seeking an experienced Case Management Specialist with excellent communication and customer service skills to work in our Downtown Los Angeles Headquarters.  Provide high quality Patient Case Management services targeting PPLA patients.  Review each follow-up plan assigned and work the follow-up template.

Our Ideal Candidate will have the following qualifications:

    • High School Diploma or equivalent required
    • One (1) to two (2) years health care experience, preferably in a medical or public health setting required
    • Demonstrated proficiency in EPM (enterprise practice management) and EHR (electronic health records) required
    • Reliable and flexible means of transportation is required
    • Bilingual English/Spanish preferred
    • Supportive of the mission and goals of Planned Parenthood

About the position:

    • Abortion patients are cared for at each of our health centers, and in part through the administrative, support, and other non-clinical services provided at all PPLA locations, and by all PPLA employees. Supporting these critical services is an essential job duty, and a fundamental responsibility of all employees and contractors.
    • Maintain ongoing and appropriate communication with Case Management Manager and/or Case Management Supervisor
    • Review each follow-up plan assigned and adhere to the follow-up template
    • Review the Visit Summary, lab result, Med Master, Communication Template, and additional notes as applicable for each follow-up plan
    • Send Confidential Morbidity Report (CMR), if applicable
    • Make 2nd, 3rd and reminder contact attempts within specified time frame requirements
    • Make appointments in EPM for patients as needed
    • Provide medically accurate information and comply with patient notification requirements per PPFA standards, guidelines, protocols, and policies
    • Document during, or immediately after, each call with accurate and clear information
    • Maintain accurate and current documentation in EHR
    • Prepare for daily mail pick-up by packing letters and inserts into envelopes, as needed
    • Document and scan receipt of all “Returned mail”
    • Scan patient documents into charts as needed
    • Ensure the accuracy of patient information; use appropriate identifiers
    • Update patient address and phone numbers in NextGen after identification of patient confirmed
    • Notify Case Management Supervisor of any patient issues or complaints
    • Submit a Case Management Productivity report for each day worked
    • Keep up-to-date on any treatment, procedure, or policy changes as communicated during huddles, newsletters, or emails from Case Management and/or Patient Services leadership
    • Comply with PPLA’s Standards of Conduct and maintain the highest level of professional and ethical standards, acting in compliance with both the letter and spirit of all applicable laws and regulations
    • Establish an effective working relationship with internal and external customers
    • Notify the appropriate contact person/people when issues or problems arise
    • Make sound decisions and demonstrate the ability to handle stressful, sensitive, and/or challenging situations appropriately
