Planned Parenthood of Northern, Central and Southern New Jersey, Inc. (PPNCSNJ) provides comprehensive reproductive and sexual health care, abortion services, education and information to women, men and teens at 17 health centers throughout 18 counties in New Jersey.

JOB SUMMARY:
Under the supervision of the Director of Patient Navigation, will provide quality patient care in accordance with delegated responsibilities and agency policies. The Patient Navigator will work within the context of Planned Parenthood of Northern, Central and Southern New Jersey, Inc.'s (PPNCSNJ) medical policies and protocols and the State of New Jersey Nurse Practice Act. Incumbent recognizes ethical, legal, and professional issues inherent in providing patient care.

ESSENTIAL FUNCTIONS:

The Patient Navigator performs a wide range of duties, including, but not limited to the following:
1.Make confirmation calls to patients regarding future appointment dates. 
2.Educate patients on care processes.
3.Respond to patient inquiries, and assist patients with their questions/concerns.
4.Serve as a link between patient and clinic. Provide health center managers with routine briefings and updates.
5.  Follow up with the patient after their appointment is completed to ensure their expectations were met.
6.Work with Medical Admin in continuously strengthening the patient experience.
7.Provides general health supervision, health maintenance, education, and counseling to patients.
8.Re-schedule appointments for patients based on clinical triage.
9.Reviews patient medical history, identifies/documents medical concerns, and obtains consents as appropriate.
10.Educates patients about contraceptive methods, safe sex practices, risk reduction, pregnancy options counseling, other information as appropriate, and documents accordingly.
11.Assists the clinician, interprets medical recommendations, and reinforces clinician instructions with patient.
12.Works with Chief Project Officer, VP's, Center Manager and other staff to facilitate efficient clinic flow and client triage, and excellent customer service. 

13. Participates in follow-up of abnormal laboratory results, as needed i.e., Cervical Cancer screening, Sexually Transmitted Infections, biopsies.
14.Follow-up of client referrals to other medical providers.
15.Ensures proper documentation of all patient interaction including clinic visits, telephone contacts, and all follow-up activities.
16.Able to indicate an emergency protocol and assists in emergency situations as needed.
17.Assists in Quality and Risk Management activities.
18.Participate in monthly staff and other meetings as scheduled.
19.Participates in the Affiliate's efforts related to revenue cycle, which include but are not limited to: Coding properly; achieving goals for productivity; meeting or exceeding revenue targets; and expense management as budgeted.

JOB REQUIREMENTS:
Minimum Education/Licensure/Certification:
Graduate of an accredited school of nursing. Current State of New Jersey Registered Nurse license or Licensed Practical Nurse license. BLS and CPR certification required. ACLS certification, desirable.

Consideration will be given to candidates with non-clinical degrees if work history and experience include procedural and medication abortion and/or patient navigation.

Minimum Work Experience: Minimum of two (2) years of experience in an ambulatory health center. Experience in reproductive healthcare preferred. Customer service experience required.
Other Requirements: Bi-lingual in English and Spanish highly desirable. Valid driver's license. Reliable transportation to travel throughout the service area.

LANGUAGE SKILLS:
Strong spoken and written communication skills. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.  Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

MATHEMATICAL SKILLS:
Ability to work with mathematical concepts such as probability and statistical inference.  Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

REASONING SKILLS:

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret a variety of instructions and deal with several abstract and concrete variables. Ability to take decisive action. Ability to exercise independent judgment to adopt or modify methods and standards to meet variations in assigned objectives.

PHYSICAL DEMANDS/MOTOR COORDINATION:
Requires hand-eye coordination; finger dexterity, and a combination of sitting, standing, climbing, stooping, balancing, kneeling, reaching, carrying, pushing, pulling, lifting (up to 25 pounds). Must be able to write, type, use telephone and computer systems, and other office equipment.

WORK ENVIRONMENT:
Exposure to routine office conditions as well as adverse environmental conditions such as dust, temperature and noise extremes, etc. May be exposed to infectious or contagious disease.
When working remotely, must be able to establish a private, professional work space where private conversations can take place.
