Position Summary: Work in conjunction with Centralized Follow Up and Patient Services Departments to develop, implement and perform centralized follow-up management and patient navigation. Promotes and restores patients' health by completing the nursing process; collaborating with physicians and other team members; providing physical and psychological support to patients and their friends and families; providing language interpretive services, as needed, supervising assigned team members, as needed. In addition, the role will provide support and collaboration on the affiliate Centering Pregnancy Program and will assume specific responsibilities as a Patient Navigator.
 
Essential Functions: 
 Centralized Follow-up Responsibilities:
Perform lab result management as defined by PPHP Medical Standards and Guidelines (MSG) and Patient Services Procedures.

Appropriately manage lab results, patient notification and follow up as defined by Physician and Clinician approved plans based on PPHP Medical Standards and Guidelines (MSG) and Patient Services Procedures.

Provide professional and comprehensive telephonic nursing assessment, evaluation, education and advice to patients within affiliate protocols.

Document accurately, appropriately and efficiently all communication with patients.

Notify pharmacies of new prescriptions and authorizations of refills as ordered by the provider.

Follow up with PPHP contracted laboratories as needed.

Maintain thorough knowledge and understanding of PPHP’s organization in order to answer questions and direct calls correctly and appropriately.

Complete quality assurance monitoring tools as specified for the department.
                        
Clinical Responsibilities:
Provide nursing coverage at surgical sites when needed and as directed.
·         Assist in all aspects of the provision of abortion care for patients including patient throughput and support as needed. Provide counseling for patients having a surgical abortion. Provide oversight and supervision to health care assistants.
·         Ensures quality of care by adhering to PPHP standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following standards of care set by state board of nursing, state nurse practice act, and other governing affiliate regulations.
·         Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures.
·         Documents patient care services by charting in patient and department records.

Centering Pregnancy Program responsibilities:
 
Independently manages the Centering Pregnancy program including:
Designing in-person and online prenatal group education materials that are inclusive of diverse and high-risk patient populations.
§Planning, scheduling, organizing, and facilitating Centering Pregnancy including the co-facilitation of group meetings.
 Identifying prenatal patients and outreach for Centering Pregnancy
Resource management and evaluation
 Providing individual counseling and education to patients at their appointment regarding contraception, prenatal health, birth, postpartum, infant feeding and newborn care
Connecting prenatal patients to vital community-based resources, programs, social services and community organizations
Responsible for independently monitoring their site’s prenatal patient lists to: ensure completion of upcoming prenatal appointments and referral appointments, make appointment reminders, make outreach calls for missed appointments, and general follow up to care calls
Monitoring prenatal referrals to ensure that they are scheduled promptly and appropriately
 Outreaching to patients after delivery to schedule newborn and postpartum visits
Updating the prenatal information, delivery information, and postpartum registry for patients in EMR system
 Meet with pre-natal providers on a consistent basis to review patient charts and enhance care coordination.
 
Patient Navigator Responsibilities
     Organizes, schedules and manages appointments for patients to ensure they receive services in a timely manner and at a location most convenient for them based on the patient’s clinical care site preferences.
     Facilitates communication between the patient, support person(s) and/or organization(s), and healthcare providers to ensure patient satisfaction and quality of care.
     Informs patients on financial aspects of care and linking the patient to financial resources as needed.
     Works with patients to identify logistical needs to access care and links patients to appropriate resources as needed.
     Works with patients to identify other support services as needed and provides appropriate linkages.
     Accommodates interpretive language services when needed.
     Collects, tracks, and reports required program data.
     Identifies and addresses existing gaps and opportunities within existing patient navigation processes, including screening processes, to ensure highest-need patients are being identified, routed, and connected to navigational support.
     Works with external vendors to facilitate access to key services, including but not limited to: transportation and lodging
     Works closely with affiliate and patient relations staff, including but not limited to Revenue Cycle team, health center managers, training team, and others to create efficiencies and improve workflows for increased abortion access and improved patient experience.
     Establishes relationships with and serves as primary point of contact for patients regarding barriers to care and social determinants of health: economic stability/employment opportunities/education, food stability, housing stability, transportation, accessing healthcare, neighborhood and physical environment/social support networks.
     Documents patient encounters, progress, and outcomes
     Maintains strict confidentiality in accordance with affiliate policies and HIPAA.
     Participate in a team approach to patient care, being cognizant of and responsive to the needs of patients and all health center personnel, including clinicians, in the clinic setting.

Qualifications:
Experience, Education and Licensure
·         Licensed Practical Nurse with a current NYS license in good standing.
·         Must have current CPR/BLS certification.
·         Minimum 1-2 -years’ experience in Women’s Healthcare, abortion services, post-procedure room preferred.
Must be willing to orient in surgical and family planning environment to develop an understanding of clinic flow and functionality, must be willing to cover ICP shifts in Smithtown, and White Plains when needed.
Ability and desire to work in maternal and child health
 Good public speaking and facilitation skills
Ability to integrate into a fast-paced work environment.
Ability to work independently on tasks, take initiative, and be self-motivated.
Capacity to work collaboratively and communicate frequently with other team members.
Exceptional time management and organization skills
Perinatal Health Education preferred.
Current BLS Certification
Excellent computer skills and proficiency MS Office applications (Word, Excel, Outlook, Powerpoint)
Familiarity with electronic health records (EHR) technology preferred.
Bilingual Spanish/English preferred.

$28.571432.9670 an hour. The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job posting and may be modified in the future.
